Haley Veterans' Hospital has a requirement for the complete maintenance of Sterling Diagnostic equipment at the Department of Veterans Affairs Outpatient Clinic, 5201 Raymond Street, Orlando, FL 32803. This is a commercial services procurement prepared in accordance with the subparts in Federal Acquisition Regulation 12.6. A written solicitation will be issued as a Request for Quote (RFQ) 673-6-99. The duration of this contract shall be for the period of October 1, 1998 through September 30, 1999 with four (4) additional one-year option periods. Contractor shall provide all labor, materials, parts, tools, travel, and equipment necessary for complete maintenance of the following equipment in accordance with original manufacturers specifications: (1.) Sterling Diagnostic Daylight Processor Film Handling System, MDL: CDS400, SN: GLFK0565; (2.) Sterling Diagnostic Daylight Modular Unloader, SN:344W552; (3.) Sterling Diagnostic Modular Daylight System MDL: Mod Disp, SN:345BW919, 345BW920, 345BW921, 45CW361, 345CW362, 345CW363, 345CW363, 345DW531, 345DW532, 345DW533, 345EW403, 345EW404 And 345EW405; (4.) Sterling Diagnostic Scanner Interface Device, Model SID, S/N GLGK623C; (5.) Sterling Diagnostic Scanner Interface Device, Model SID, S/N GLGK635A; (6.) Sterling Diagnostic LINX Laser Printer, Model LINX LP400 S/N GLHA1528. Successful offeror shall be able to obtain any manufacturer specific software and/or parts in order to provide for total maintenance of each model of equipment described herein. Successful offeror shall show evidence of successful completion of service training program for all equipment specified in this contract. Standard Industrial Classification Code is 7629 and the business size standard is $5.0 million. Issue date of solicitation is on or about July 15, 1998.
