Q -- PSYCHIATRIST SOL RFQ 124-0018 DUE 073198 POC Richard Houser, Contract Specialist 608-584-6306 The Federal Correctional Institution, Oxford, WI is soliciting to establish a contract with a Psychiatrist to provide psychiatric care, treatment, and evaluation of the mental health needs and psychiatric disorders of the inmates incarcerated at the Federal Correctional Institution and Federal Prison Camp. The contractor will provide written and/or dictated reports for all patients referred for psychiatric evaluation to the Clinical Director, includiing recommendations for further diagnostic testing and/or diagnostic treatment. Serives to be provided in two hour sessions, eight sessions per month for an estimated 98 sessions for the contract period of October 1, 1998 through September 30, 1999. The offeror must have completed residency in psychiatry and must possess licensing for the practice of medicine in the State of Wisconsin to be considered for award. Offerors must have verification of credentials; board certifications, proof of professional liability insurance, copy of diploma(s) for credentialing pursuant to JCAHO standards and Bureau of Prisons Policies. A firm-fixed price contract will be awarded on or about August 7, 1998 to the lowest qualified bidder. This acquisition is 100% set-aside for small business.
