99 -- TERMITE INSPECTION OF MILITARY FAMILY HOUSING UNITS SOL F0260498Q0512 DUE 071498 POC Joe Chapman 602-856-7179 x234 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, No. F02604-98-Q0512 is issued as a request for quotation (RFQ). This solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-4. This acquisition is reserved for small business; SIC: 7432; Size Std.: $5,000,000. Contract Line Item (CLIN) 0001; Nonpersonal services to include all labor, parts, materials, transportation, and equipment necessary to inspect Military Family Housing units for termites as described herein. Termite inspection to be conducted via termite-sniffing dogs. All dog handlers shall be certified in category "C", Wood Destroying Organisms. Copies of state certifications shall be included in bid package. Prospective offerors shall provide references evidencing their effectiveness, with at least two years experience. Method of inspection should be as follows: A visual interior and exterior inspection shall take place with dog handler and dog. An intensified inspection, verifying whether or not termites are present shall take place at any time the dog is alerted. The exterior inspection shall include the perimeter of the house and the interior inspection shall include the entire housing unit, garage area and any storage areas. Upon completion of inspection, a comprehensive written report will be provided by the contractor including a diagram locating any area of infestation, any area with conditions conducive to infestation, and recommended corrective actions. Prospective offerors must warrant their inspection for a minimum period of 6 months. If infestation is identified within the warranty time period, extermination will be the responsibility of the contractor. Desired delivery : FOB: Destination to Luke AFB, AZ.
