Payment for services will be on a monthly capited rate basis. The contract term will be one base year with two 1 year options for contract extensions. The primary health care services will be provided in Craighead County Arkansas (Jonesboro) area. Contractor is expected to meet or excced JCAHO requirements for Primary Care. Facitilities must be in complience with the American Disability Act requirements. The VA is expecting to enroll approximately 1,000 patients within one year after award. The contractor shall provide all personnel, clinic facilities, equipment, supplies, and supervision for the provision of primary health care services. The contractor will be responsible for the provision of all radiological and laboratory services associated with primary care services. The contractor must have a primary care provider on call 24 hours a day, 365 days a year to respond to enrolled pateint telephone inquires. The VAMC will make an award to offeror that represents the best value to the VAMC. Contract award will be based upon the evaluation of offers using selection factors including cost as one facor.
