Item 001: Lighting Fixtures, WIDELITE Part Number ILEM-1000-DTG-QV-1-FBL50-BGP-H47P-1, including Lamps, 1000 Watt Metal Halide, Mogul Base, Clear, No Substitutions. Quantity 60 each. Delivery is FOB Destination to the Bureau of Reclamation, Hoover Dam Warehouse, Boulder City, Nevada, 89005. Offers may be submitted on the SF-1449 or letterhead stationery to Bureau of Reclamation, P.O. Box 61470, LC-3132, April Webb, Boulder City, Nevada 89006. Facsimile offers are acceptable in accordance with FAR 52.215-18 to fax number 702-293-8050. The Government will NOT be responsible for any failure attributed to the transmission or receipt of their quotation. As a minimum, offers must show the following: 1) the solicitation number; 2) the name, address, and telephone number of the offeror; 3) A description of the items being offered in sufficient detail to evaluate compliance with the requirements in this solicitation; 4) Price and discount terms; 5) A completed copy of the representations and certifications at FAR 52.212-3.
