FAR Clauses 52.212-1, 212-3, 212-4 and 212-5 will apply. RFQ will be accepted by this medical center for award of a contract to purchase a; (1) Diagnostic & Therapeutic System that employs invasive procedures involving the spine and treatment of pain, (2) Epiduroscope with Fiberoptic to be used with the system offered . The solicitation will include Bid option to provide consumables at a firm fixed price for one (1) year. As a minimum the system shall consist of: (1) Video Console Camera & Light Source (2) Endocoupler & Video Camera with cable lead (3) 19" Medical Grade Monitor (4) VHS Video Recorder (5) Color Video Digital Printer. As a minimum the consumable shall consist of : (1) Video Guided Catheter with a channel for the epiduroscope and a working channel. 2) Epiduroscopy Procedure Kit 3) Introducer Kit-Dilator and Sheath 4) Floppy tip guidewire, all consumable must be compatible with the Diagnostic & Therapeutic System offered.
