R -- MAILING AND DISTRIBUTION SERVICES SOL RFQ 618-144-98 DUE 071498 POC Mavis Nelson 612-725-2189 Under Statutory Authority 41 U.S.C. 253 (1) the Government anticipates entering into sole source procurement negotiations with Scanning Concepts, Minneapolis, MN for VISN 13 network-wide survey of veterans. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, reference RFQ 618-144-98; a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through FAC 97-05. SIC code 7389 applies. Size Standard is $5 million. Department of Veterans Affairs is seeking: all labor and material from vendor experienced with the forms development and existing VA-owned scanning software used the past 4 years and continuing to be compatible for future projects; experienced with the printer alignment features and requirements of the software; experienced with canning interface imbedded in the forms design software; guarantee that forms received past the completion of the end of the contract can be scanned by VAMC, Minneapolis using existing software; use a single version of software that creates, prints and scans forms and questionnaires such that a continuous stream of data may be maintained in the format specified by VAMC; prepare 2 pages double sided printing and cover letter, reproduce, and mail survey to approximately 80,000 veterans, with a second mailing to non-responders (est. 40,000), for 3 subgroups within the entire group, with 1000 patients selected to receive an additional single page, second survey; responsible for taking forms developed at VAMC, printing, encoding unique patient identifier, printing cover letters, mailing, receiving returned surveys, mailing out second survey to non-responders, scanning all forms, and provide data files with all survey data to VAMC in format specified by VAMC; VAMC will provide template forms and a disk of names and mailing addresses of all patients in network (confidentiality to be maintained); use Forms-On-Command, version 5.0, Windows based software to create, print and scan an OMR form survey; must be Year 2000 compliant; complete project within 80 calendar days ARO Notice to Proceed; be available for consultation an additional 90 calendar days at no additional cost to the Government; standard commercial industry terms and conditions. The above authority is cited and justification made based on the determination that the Government's minimum needs can best be satisfied by services available from one source. This contract action is for supplies and services for which the Government intends to solicit and negotiate with only one source under FAR 6.302. Other persons may identify their interest and capability to respond to this requirement by July 14, 1998. This notice of intent is not a request for competitive proposals. However, all interests received on or before July 14, 1998 will be considered by the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
