Q -- OPTOMETRISTS SOL 124-0022 DUE 082098 POC Ronn Lindert, Contracting Officer, 608-584-6119 The Federal Correctional Institution, Oxford, WI is soliciting to establish a contract with two optometrists to provide specialty medical services in the area of optometry and ocular (eye) needs of inmates incerated at the federal Correctional Institution located in Oxford, WI. Offeror to provide care, evaluation and treatment of ocular disease, the prescribing of eyeglasses and availability of glasses for purchase if patients so chose. Offerors will provide full optometric care, provide medical medical administrative with prescriptions for eyeglasses and/or written reports of consultations referred by the institution medical staff for each individual inmate seen. Services to be provided in two hour sessions, one session per week per optometrist, for an estimated 51 sessions per contract for the contract period of October 1, 1998 through September 30, 1999. Offerors must be a graduate from an approved school of Optometry with current State of Wisconsin licensure and/or certification to be considered for award. Offerors must have valid liability insurance issued by a responsible carrier in order to be eligible for award. There will be a pre-bid conference/site visit at the Federal Correctional Institution on August 6, 1998. A firm fixed-price contract will be awarded on or about September 8, 1997 to the lowest qualified bidder. This acquisition is 100% set-aside for small business. All responsible sources may submit a quotation which shall be considered by this office. The estimated range of this agreement is $25,000 to $100,000.
