SOL Request for Quotations 224-0004-9 DUE 082198 POC Patricia Jones, 925/803-2780 The Federal Bureau of Prisons, Western Regional Contracting Office, 6600 Goodfellow Street., Dublin, California 94568 has a requirement for an offeor to provide Phlebotomy Services. The services are quried to be provided to the Inmate population at the Metroplitan Detention Center, 535 N. Alameda Street., Los Angeles, CA 90012. All services provided shall be performed in accordance with standards and methods generally accepted within the Contractor's field of expertise, inlcuding but not limited to, the Statement of Work, as cited in the Request for Quotation. A general overview of the duties required that, the offeror be able to maintain laboratory logs, perform scheduling of patients, perform proper patient instructions, perform phlebotomy (drawing of blood), speciment collection and ensure that results are properly transferred to staff physicians for review. The Offeror must be a Certified Phlbotomist,/Laboratory Technician or qualified Medical Assistant, a graduate of an recognized Phlebotomy/Medical Assistant school, possess valid proof of malpractice insurance (Insurance compatible with the local community, where the services are to be performed), the offeor must be familiar with OSHA guidelines regarding bloodborne pathogens, possess valid CPR certification, and be computer literate (Windows 3.1/ Windows 95). The services shall be required five sessions per week, 52 weeks per year, Monday through Friday, except Federal Holidays. One session shall consist of five hours. Services shall be provided betwee the hours of 8:00 a.m. and 2:00 p.m. or as required by the Hospital Administrator at the Metroplitan Detention Center. All Offerors are advised that their quotation shall be evaluated in accordance with the evaluation factors as cited in the Request for Quotation. The anticpated period of contract performance will be : Base Year: Date of Award through 12 months from award date; Option period #1: 13 months from date of award through 24 months from date of award; Option period #2: 24 months from date ofaward through 36 months from award date; Option #3: 37 months from date of award through 48 months from award date; Option period #4: 49 months from date of award through 60 months from award date. Proposal packages will appximately be available 7/24/98 with an approximate option date of 8/21/98. All offerors are advised that the resultant contract will be subject to the wage rates as determined by the Department of Labor.
