The Albuquerque Area Indian Health Service Office to solicit proposals from prospective offerors for providing Readiologist Services for the Acoma-Canoncito-Laguna Service Unit, located on the Acoma Indian Reservation, San Fidel, New Mexico. The contract provider shall provide clinic visits on Thursdays of each week during the contract year. Total not to exceed 53 clinic visits. Services to include but not be limited to: Interpretation of routine diagnostic x-rays and ultrasound studies; performance and interpretation of fluoroscopic studies; interventional procedures in radiology diagnosis, including invasive techniques; and other standard radiology services. Contractor to show evidence that he/she is Board Certified in Radiology and State Licensed from any one of the 50 states with the U. S. A., possess a degree from an accredited School of Radiology, must have maintained and upgraded clinical skills through meetings, seminars, workshops and provide proof of current medical liability coverage. A non-personal service contract will be awarded for a base year and four, one-year options for renewal. A requirements type contract will be awarded to the most responsive and reasonable offer. Individual delivery orders to be issued against the contract upon availability of funds. The Government will accept and consider all offers received. Technical evaluations will be conducted based on written technical evaluation factors and their assigned numerical weights. All prospective offerors are advised that the technical and cost proposals are of equal value. In the case of tie, cost/price factors will be the determing factor for award.
