S -- THERAPEUTIC POOL MAINTENANCE SOL RFP672-180-99 DUE 090398 POC POC Rey F. Clemente (787) 766-5389 This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. I. The solicitation number is RFP672-180-99. II. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-03. III. This is a 100% Small Business Set-Aside. The standard industrial classification code is 7389 and the small business size standard is $5.0 M. IV. Provide a Service Contract to furnish all necessary Labor and Maintenance of THERAPEUTIC POOL located at VA Medical Center, (VAMC), first floor RM# B-145, San Juan, Puerto Rico. The contract shall be for a Base Year (from 10/01/98 to 09/30/99) to include four (04) additional option years: Option period I (from 10/01/99 to 09/30/2000) Option period II (from 10/01/2000 to 09/30/2001) Option period III (from 10/01/2001 to 09/30/2002) Option period IV (from 10/01/2002 to 09/30/2003). Cleaning requirements: Contractor shall sweep and mop this pool with a disinfectant solution once a week. All work shall be performed between the following schedule: Tuesday and Thursday only from 5:00 a.m. to 8:00 a.m. Pool size is as follow: 40" X 30" with a depth ranging from 3' to 5'. The government will provide all supplies and materials as part of term and condition of this contract. Contractor shall report directly to room C-26 for sign in/out. Service Reports: The Service Technician shall submit each service report to the Contracting Officer Technical Representative (C.O.T.R) or his/her designee at the conclusion of each service performance. Offeror shall submit price for base year and optional years. Offerors shall have and submit with their offer all applicable licenses and permits required by the Federal Government, and Commonwealth of Puerto Rico. Evaluation of options shall not obligate the Government to exercise the option(s). Provision 52.212-1, Instructions to Offerors -- Commercial Items, is applicable. Clause 52.212-2, Evaluation- Commercial Items, The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The factors shall be used to evaluate offers: I. Technical and Past Performance, are significantly more important than cost or price in accordance with FAR 15.304. Offeror must submit a completed copy of provision 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its proposal. Clause 52.212-4, Contract Terms and Conditions -- Commercial Items is applicable to this acquisition. Clause 52.212-5, Contract Terms and Conditions, Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition.
