synopsis/solicitation for a commercial item prepared in accordance with
the format in Subpart 12.6, as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; proposals are being requested and a written solicitation
will not be issued. The solicitation number for this Request for
Proposal (RFP) is N00244-98-R-0079. This solicitation and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-04. This solicitation is issued on an unrestricted basis,
under Standard Industrial Code 7629 with a small business standard of
$5.0 million. FISC San Diego has a requirement for services, material,
and equipment necessary for on-site preventive and corrective
maintenance/service of the Government-owned Hydrajust III Tomography
Systems manufactured by Liebel-Flarsheim located at the Naval Medical
Center San Diego (NAVMEDCENSD), Urology Department. Contract will be
for one (1) base year and four (4) option years. This requirement is
for a fixed price contract for thirty-five line items. Line items will
use the following format -- Line Item Number, Description/Model,
Serial, BCN, and Location. Base year (01 OCT 1998 thru 30 SEP 1999)
consists of Line Items 0001-0007: 0001, Hydrajust III,
333UBD-IATXS-2154M2, 11795, Bldg.3-2 Room 1; 0002, Hydrajust III
Tomography System, 33UBD-IAFXS-2211M2, 11796, Bldg.3-2 Room 2; 0003,
Hydrajust III Tomography System, 33UBD-IAXTS-2215M2, 11797, Bldg. 3-2
Room 3; 0004, Hydrajust III Tomography System, 333UBD-IAXTS-2256M2,
11798, Bldg.3-2 Room 4; 0005, Hydrajust III, 333UBD-IAXTR-2251M2,11799,
Bldg.3-2 Room 5; 0006, Hydrajust III Tomography System,
3333UB-IAXTXR-2260M2-R, 11800, Bldg.3-2 Room 6; 0007, Hydrajust III
Tomography System, 333UB-IATXS-1265M1, 11801, Bldg.1-4 Room 9. Option
year one (01 OCT 1999 thru 30 SEP 2000) consists of Line Items 0008
thru 0014. Option year two(01 OCT 2000 thru 30 SEP 2001) consists of
Line Items 0015 thru 0021. Option year three(01 OCT 2001 thru 30 SEP
2002) consists of Line Items 0022 thru 0028. Option year four(01 OCT
2002 thru 30 SEP 2003) consists of Line Items 0029 thru 0035. All
Option years require the same type of service and maintenance agreement
as the Base year Line Items 0001-0007. Inspection and Acceptance shall
be at destination. All items are to be delivered F.O.B. Destination.
Medical Equipment Service Maintenance Statement of Work -- General
Requirements: The effort required hereunder shall be performed in
accordance with Statement of Work and in accordance with all other
terms and conditions set forth herein. The contractor shall provide all
services, materials, and equipment necessary for the repair/service of
the above equipment to ensure dependable and reliable operation of the
equipment. The scope of work performed under these specifications
includes the furnishing of all labor, equipment, materials, and parts
to perform all repairs on equipment listed, to assure continued
operation at their designed efficiency and capacity. The contractor
shall perform the following functions: Perform service maintenance to
industry standards. Ensure that only FACTORY AUTHORIZED FIELD ENGINEERS
and TECHNICIANS who have gone through original equipment manufacturer
(OEM) or comparable third party service schools for the above mentioned
equipment, shall be employed in the performance of any and all work
performed under this contract. Upon request, the certificates (or
notarized copies) will be provided to the Medical Repair Branch for
verification. The highest standard of professional capability and
electrical/mechanical workmanship is to be maintained throughout the
life of this contract. Make repairs to the extent necessary (as
determined by inspection tests or disassembly) to ensure a functional
system that will efficiently serve its intended purpose. Correct
inoperable condition in a timely manner including loaner response time
of no later than (24) twenty-four hours after telephone notification,
to a monitored contractor, to NAVMEDCENSD, Medical Repair Branch and
Purchasing Division immediately upon contract award. Provide only work
necessary to restore the equipment to a serviceable/ operating
condition by adjustments, replacement parts, or minor repairs when it
is determined that extensive repairs and parts replacement are not
necessary. Equipment improvements/modifications shall be made only on
written approval and direction of the NAVMEDCENSD Medical Repair
Branch. Notify the NAVMEDCENSD Medical Repair Branch immediately upon
receipt of OEM or replacement parts/equipment safety recall notice.
Insure that original design and functional capabilities will not be
changed, modified, or altered unless such changes are authorized by the
NAVMEDCENSD Medical Repair Branch. Provide suitable modern recommended
repair equipment/tools required for the satisfactory execution of all
repairs made. Furnish manufacturer OEM approved lubricants and
lubricate wear points within equipment. Extend to the Government all
commercial warranties on replacement parts, consistent with standard
industry practices. Maintain an adequate spare parts inventory on parts
with a history of high failure/replacement rates. SPECIAL CONDITIONS:
The contractor shall send a loaner unit within 48 hours of receiving
call, paying postage on loaner. Repair of Government-Owned equipment
will be accomplished at factory. Units will be returned to NAVMEDCENSD,
repaired if possible with contractor paying for postage. TITLE TO
EQUIPMENT: The contractor will not assume possession or control of any
part of the equipment. The Government retains ownership to title
thereof. LIABILITY: The contractor shall not be liable for any loss,
damage, or delay due to any cause beyond his reasonable control
including but not limited to acts of government, strikes, lockouts,
fire, explosion, theft, floods, riot, civil commotion, war, malicious
mischief or acts of God. UTILITIES: The contractor may use Government
utilities (electrical power, compressed air, and water) that are
available and required for any service performed under this contract.
Contractor electrical equipment must be approved by the building
engineer to ensure compatibility with NAVMEDCENSD electrical wiring and
equipment. ACCESS TO EQUIPMENT: The contractor will be provided
reasonable access to all equipment which is to be serviced and utility
outlets required to do the service. The contractor will be free to
start and stop all primary equipment incidental to the operation of the
maintained equipment after permission is received from on duty
personnel responsible for such equipment. FIELD SERVICE REPORTS: The
contractor or his representative is required to report to the Medical
Repair Branch during the hours of 730-2200 Monday through Friday, prior
to and upon completion of any service/ repair performed. The contractor
or his representative shall furnish to the NAVMEDCENSD Medical Repair
Division a legible copy of the Field Service Report upon completion of
work performed. The contractor or his representative shall complete
the Government copy of this Field Service Report by including the
following: Date and Time notified; Date and Time of arrival; BCN, Type,
Serial # and Model # of equipment; Time spent repairing/servicing;
Description of malfunction, General Description of replaced parts and
services performed; Comments as to cause of malfunction. GOVERNMENT
PERSONNEL: NAVMEDCENSD employees will not perform maintenance or
attempt repairs to equipment while such equipment is under the purview
of this contract unless agreed to in writing by the contractor. PARTS
AVAILABILITY: To ensure minimal downtime to equipment, the contractor
shall maintain replacement repair parts and materials necessary to
perform each repair or supply said parts and materials within (1) one
day. COMPENSATION: All compensation for labor, parts, and materials is
included in the contract price. Parts and supplies provided under this
contract shall be guaranteed to be equal in all respects, including
performance, interchangeability, durability, and quality to the OEM
parts when new or as presently recommended by the manufacturer.
PREVENTIVE MAINTENANCE AND CORRECTIVE MAINTENANCE SCHEDULES: Preventive
maintenance shall be performed four(4) times per fiscal year with
actual months of service to be designated by the NAVMEDCENSD Biomedical
Repair after contract award. Corrective maintenance shall be performed
Monday -- Friday, 0800 -- 1700 hrs. The following clauses and
provisions are applicable and hereby incorporated by reference and will
be provided by the Contracting Officer upon request. FAR 52.212-1
Instructions to Offerors -- Commercial Items (Jun 1997); FAR 52.212-2
Evaluation -- Commercial Items (Oct 1995), with paragraph (a) completed
as follows: "(i)Ability to meet required response times/maintenance
schedules and certification requirements for field engineers and
technicians. (ii) Price. Ability to meet required response
times/maintenance schedules and certification requirements for field
engineers and technicians represents the MINIMUM criteria for an
"acceptable" offer. Failure to meet this criteria will render the offer
"technically unacceptable" and the offer will not be considered for
award. Response times, maintenance schedules and certification
requirements are set forth in the Statement of Work. Ability to meet
required response times/schedules shall be demonstrated by the
offeror's submission of a signed offer which has not taken exception to
these requirements. Offerors are to submit the certificates of course
completion for all field engineers and technicians planned to work
under the contract with their proposal. Offers which take exception to
these requirements will be determined to be "unacceptable" and not
considered for award. Award will be made to that responsible Offeror
who meets the above minimum schedule/certification requirements and
offers the lowest price." Option years will be included in the
evaluation of the proposal price. FAR 52.212-4 Contract Terms and

total duration of this contract, including the exercise of any options
