304/285-5883 THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL
ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS
SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS
ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; BIDS ARE BEING
REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This
solicitation is issued as Request for Proposal No. RP98-035. This
procurement is being issued unrestricted. The Standard Industrial
Classification is 3826 with a size standard of 500 employees. This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-04. Furnish and
install at the National Institute for Occupational Safety and Health
(NIOSH), 1095 Willowdale Road, Morgantown, WV one each of the following
five Contract Line Items (CLINs), brand name or equal to Packard
Instrument Co. (CLIN 001) Model A307 Sample Oxidizer System; (CLIN 002)
Oximate 80 System; (CLIN 003) Oxidizer Starter Kit; (CLIN 004) #307
Service manual and (CLIN 005) second year warranty. Offers shall comply
with the following SPECIFICATIONS REQUIRED FOR TISSUE SAMPLE OXIDIZER
WITH ROBOTICS: CLIN 001. Tissue Sample Oxidizer: handles samples up to
1.5 grams; combusts single and dual radiolabel samples containing
Hydrogen 3 and/or Carbon 14; radionuclide recovery for both Hydrogen 3
and Carbon 14 of greater than 97%; radionuclide memory of less than
0.08% for Hydrogen 3 and Carbon 14 labeled samples; does not require a
catalyst; can accommodate wet, dry, or freeze-dried samples; automatic
vial positioning, Oxygen control, sample combustion, reagent
dispensing, cleaning cycle, and basket unloading; high temperature
flame combustion up to 1300 degrees C; splash-free system; collect
tritium activity for 10-15 grams of solid material in a single vial;
accommodates 20 ml glass or polyethylene vials; capacity to process up
to 60 samples per hour for each nucliotide; pumps adjustable from 0 to
18 ml. CLIN 002. Tissue Sample Oxidizer robotics: 2 sample trays, 40
samples each; 1 sample tray, 100 vial capacity; 240 sample per day
throughput; automated vial uncapping and capping; automated Combustaid
addition; multiple combustions collected in the same vial; sensor
controlled safety shut off. DELIVERY SHALL BE NO LATER THAN 60 DAYS
AFTER RECEIPT OF CONTRACT. Evaluation Factors: Offers will be evaluated
on their ability to meet or exceed the stated specifications and
demonstrated past performance on contracts of similar size and
complexity to this requirement. Technical proposals and cost or price
are approximately equal in importance. The Government intends to make
a single award to the responsible offeror whose proposal conforms to
all requirements of the solicitation and is considered the best value
to the Government. Offers shall be proposed based on F.O.B. Destination
(Morgantown, West Virginia) and the requirements at FAR 52.214.21
"Descriptive Literature" apply. Information provided must demonstrate
the unit's ability to meet the specification or your offer may be
rejected. The following FAR provisions apply to this solicitation: FAR
52.212-1, "Instructions to Offerors -- Commercial Items"; 52.212-4,
"Contract Terms and Conditions -- Commercial Items" and 52.212-5
"Contract Terms and Conditions Required to Implement Statutes or
Executive Orders -- Commercial Items." Additionally, the following FAR
clauses cited in Clause 52.212-5 are also applicable to this

PHS, CDC, NIOSH, ALOSH, Attn: Brenda Goodwin, 1095 Willowdale Road,
Morgantown, WV 26505: (1) original and three copies of a completed SF
1449, signed by an individual authorized to bind the organization; (2)
Schedule of Offered Supplies/Services containing pricing of Contract
Line Items (CLINs) 001 through 005 and published price lists (if
available); (3) completed Representations and Certifications --
Commercial Items, FAR 52.212-3; (4) descriptive literature, tables,
etc. which demonstrate compliance with specifications for evaluation
purposes; (5) Past performance information (include names and addresses
of Company/ Government Agency, name and telephone numbers of customer
contacts along with contract number, and description of work performed
with your offer.); and (6) acknowledgment of solicitation amendments,
if any. Facsimile Bids are not authorized. Offers that fail to furnish
the required information, or reject the terms and conditions of the
solicitation may be excluded from consideration. Award of a one firm
fixed-price contract is anticipated. Information regarding this notice
