Naval Academy has a requirement for a commercial service -- Civilian Employee Assistance Services for base period of 1 October 1998 through 30 September 1999 and two option years. The awardee shall maintain a facility within a 10 minute walk of the U. S. Naval Academy, Annapolis, MD 21402 and provide a 24 hour answering service, 7 days a week. At a minimum the awardee shall provide additional counseling sites at various locations in: (1) Anne Arundel County; (2) Queen Anne's County; (3) Baltimore County; (4) Baltimore Metropolitan Area in the State of Maryland; and (5) Newport, Rhode Island. Emergencies shall be handled immediately and may require the awardee to provide on-site services at any organization served. Criminal background checks are required of all awardee's employees. These employees shall complete a DD Form 398-2: DOD National Agency Questionnaire and DD Form 25B: FBI Applicant Fingerprint Card. A criminal history background check shall be conducted through the State CriminalHistory Repositories on employment and security applications. An Installation Record Check shall be completed only on those employees who have a DOD affiliation such as living or working on an installation or is an active duty member or family member.
