Furnish the services of cardiopulmonary perfusionist at the VA Medical Center, Portland, Oregon, for the base year beginning October 1, 1998, through September 30, 1999, with two (2) one-year renewal options. The perfusionist shall be responsible for: (a) setting up and operating the perfusion and related equipment and perform associated duties and responsibilities; (b) maintaining a complete record of perfused fluids on by-pass as well as other required records relative to perfusion activities; (c) responsible for setting up and operating the balloon pump and associated duties and responsibilities; (d) provide response to emergency calls within forty-five minutes after notification; (e) provide stand-by service, in-house, with cardiopulmonary perfusionist available in operating room with bypass unit in readiness. Contractor shall be Board certified or Board eligible by the Amercian Board of Cardiovascular Perfusion. Perfusionists will be required to be on site during normal working hours in the operating room. The perfusionist must be available during other than normal working hours on an "as needed" basis. Services during liver transplants may be required. A request for quotation of a commercial service will be issued on or about August 7, 1998.
