This is a combined synopsis for commercial items prepared in accordance with subparts in 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation RFQ 581-16-98. The SIC is 3571 and the small business size is 1000 employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-07. The VA Medical Center, Huntington, WV intends to negotiate a sole source procurement with Prologix Inc., 103 Roack Road, Horsham, PA. Miscellaneous items from various manufactures with custom modifications and all are integrated into a configuration for the CPRS SYSTEM, Fujitsu PT510, w/900 MHz Aironet radio, 1.6 GB hDD 32 MB RAM, FMWKB1A -- Keyboard, Health care Cart, Port Replicator -- FMW26CR3, Floppy Disk -- FMWFD2, CD-ROM, SC-1017 17.7 Transmobile Workstation, LT-DC 1000 Transmobile Workstation. If submitting a name on brand name or equal, adequate technical information shall be submitted with the quote for the Government's complete evalaution. Failure to submit adequate technical information shall include but not be limited to: technical specifications, product availability, product picture, product features, etc. Quotations shall be submitted on "all or none" basis. Partial quotations shall be rejected. All quotations shall state delivery time, FOB Destination. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certification-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5, and 852.210-77 also apply. Quotations shall be submitted to the attention of Mike Hughey (90C). If no feasible responses are received by 2:00 PM local time August 26, 1998 a procurement will be named with the above named-source.
