This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. F2660098Q2036 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-05. Solicitation is reserved for small business concerns. The SIC for this solicitation is 7699. The small business size standard is $5 million in annual gross revenue for the previous three fiscal years. Item description as follows: Non Personal Service to provide all labor, tools, parts, materials, facilities, and transportation necessary to provide quarterly preventive maintenance inspections for (16) anesthesia units located at the Mike O' Callaghan Federal Hospital, N. Las Vegas NV. All Work shall be done in accordance with the statement of work. The period of the contract is from 1 Oct 98 to 30 Sep 99 with (4) four option years. There are (20) twenty line items included in this solicitation (see bid schedule). The provision at FAR 52.212-1, Instructions to Offerers-Commercial, applies to this solicitation. Award will be made to the responsible, responsive offeror whose offer will result in the lowest technically acceptable price. The government will evaluate options for award purpose by adding the total price for all options to the total price for the basic requirement for the total five-year period. The government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the government to exercise the options.
