is a combined synopsis/solicitation for commercial items, prepared in
accordance with the format in Federal Acquisition Regulation (FAR)
subpart 12.6 as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation:
quotations are being requested and a written solicitation will not be
issued. This solicitation is issued as a request for quotation (RFQ).
Submit written quotes (oral offers will not be considered), on
requisition F41636-98-Q-0742. This procurement is restricted to Small
Business only ($5 million). This solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-04. Standard Industrial Code 8082. The provisions at FAR
52.22-41 Service Contract Act of 1965, as amended (May 89) applies to
this solicitation. The employee class is 12071, Licensed Practical
Nurse I, with 29.55% fringe benefits. U.S. Department of Labor, wage
determination number 94-2521 revision 16 (06/01/1998) is applicable to
this requirement. Contractor shall perform nonpersonal service to (1)
Provide Licensed Practical Nursing support services. Contractor care
shall be provided at Lackland Air Force Base, TX. The quality of care
provided shall meet or exceed the standards of professional nursing
health care as identified by the Joint Commission of Accreditation for
Hospital Organizations. (2.1) Contractor personnel shall present a
neat appearance and be easily recognized as Contractor employees.
Personnel shall wear a Contractor provided name tag with the company
name or phrase "Contract Nurse", individual's name and specialty
displayed. (2.2) Possess a valid license to practice as a practical
nurse or vocational nurse from a United States jurisdiction. (2.3)
Shall be CPR certified. (2.4) Completed a program in an accredited
school of nursing acceptable to the Surgeon General, United States Air
Force. (2.5) Personnel shall have 12 months experience, in the last 24
months, of working with medical/surgical patients, preferably with
rehabilitation patients or orthopedic patients. (2.6) Contractor shall
provided written verification of qualifications, licensure and
certification upon request of the Contracting Officer (CO) or
authorized representative. (2.7) Contractor personnel shall be current
on all immunizations, including tuberculosis and shall have received
a Hepatitis B vaccine, sign a voluntary declination, or have documented
proof of immunity to Hepatitis B infection. The Contractor shall
provide to the CO, upon request, certification that the Contractor
personnel have completed the medical evaluation required. (3) Routine
services shall be provided Monday through Friday, from 0900 to 1300,
excluding Federal holidays (New Year's Day, Martin Luther King, Jr's
birthday, President's Day, Memorial Day, Independence Day, Labor Day,
Columbus Day, Veteran's Day, Thanksgiving Day, Christmas Day). (3.1)
Contractor notification will be provided by the Government at least 2
hours in advance of a scheduled duty day when services will not be
required for the day. (4) The provisions of FAR 52.237-7,
Indemnification and Medical Liability Insurance (Jan 97) applies. (a)
$1 million per occurrence and $3 million annually. (4.1) Contractor
shall provide to the CO, upon request, evidence of insurance
documenting the required coverage. (5.1) Performs a variety of nursing
care activities and responsibilities in the care and treatment of a
handicapped (quadriplegic) Federal employee, as well as health care
maintenance. (5.2) Provides intermittent catheterization, to include
preparing patient for catheterization, inserting and removing catheter,
draining and disposal of wasted, and redressing of patient.
Catheterization shall be provided on an as needed basis. (5.3) Assists
in patient feeding, to include preparation and cleaning of eating
area. (5.4) Assists patient, as needed, in bowel evacuation procedures.
For the performance period of 1 Oct 98 to 30 Sep 99 in accordance with
the above specifications. Contractor must bid on all items. FOB:
Destination for delivery is 37 TRSS/ATT, Lackland AFB, TX. 78236. The
provisions at FAR 52.212-1, Instructions to Offerors -- Commercial
Terms, applies to this acquisition. The clause at FAR 52.212-5,
Contract Terms and Conditions Required to Implement Statues or
Executive Orders -- Commercial Items, applies to this solicitation.
Specifically the following clauses, cited are applicable to this
solicitation : FAR 52.222-26, Equal Opportunity; FAR 52.222-35,
Affirmative Action for special disable and Vietnam ERA Veterans; FAR
52.222-36, Affirmative Action for Handicapped Workers. The clause at
DFARS 252.212-7001, Contract Terms and Conditions required to implement
Statues or Executive Orders applicable to Defense Acquisition of
Commercial Items, applies to this solicitation. Specifically the
following clauses, cited are applicable to this solicitation: DFARS
252.225-7001, Buy American Act and Balance of Payment Program. Offerors
must include with their offer a completed copy of the provisions atFAR
52.212-3, Offeror Representations and Certifications -- Commercial
Items, and DFARS 252-212-7000, Offerors and Representations and
Certifications -- Commercial Items, applies to this solicitation. FAR
52.245-4, Government-furnished Property. Also applicable is DFARS
252.204-7004, Required Central Contractor Registration (CCR). CCR can

award can be made. Award will be based on evaluation of: meeting
