B -- FLY SAFE! A HISTORY OF AVIATION SAFETY IN THE AIR FORCE, 1908-1998 SOL F49642-98-R0218 DUE 082898 POC POC Ms Sharon Hinds, Contract Specialist, or Janice K. McConaha, Contracting Officer This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This Solicitation Number, F49642-98-R0218, is issued as a Request for Proposal (RFP) and will result in a Firm-Fixed Price (FFP) contract. Period of performance will be one 12-month base year and two one-year option years. Funding for the base year is up to $65,000. This acquisition is being acquired on a competitive basis and as a 100% small business set-aside. FAR 52.219-6, Notice of Total Small Business Set-Aside, is hereby incorporated by reference. The Standard Industry Code (SIC) is 8999; The size standard is $5 Million. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-05. The SF 1449, Schedule B, Section C-Statement of Work (SOW), DD Fm 1423-Contract Data Requirements List (CDRLs), Central Contractor Registration letter dated 22 Jun 98 and Past Performance Ratings are attachments 1 -- 6, respectively, to this notice. Atch 7 is an addendum to this notice which sets forth mandatory technical requirements. Ratings for technical requirements will be Acceptable: meets mandatory requirements or Unacceptable: Fails to meet mandatory requirements. A proposal found deficient in the technical requirements will be considered unacceptable and rejected in its entirety. Contractors shall obtain all attachments cited herein via fax (202-767-7814) or E-Mail (shinds@mail.bolling.af.mil) request from the Contract Specialist. Request should be made immediately; confirmation of receipt of a legible fax is required. In order to be considered responsive, offerors must certify in writing that their proposal meets all requirements set forth in this solicitation, attachments included. STATEMENT OF WORK: Complete a history of aviation flying safety entitled "Fly Safe! A History of Aviation Safety in the Air Force, 1908-1998". The SOW (Atch 3 to this notice) may be requested as stated above. Deliverables will be submitted to, and accepted by, the Contracting Officer's Technical Representative (COTR) at AFHSO/HO, 500 Duncan Ave, Box 94, Bolling AFB, DC 20332-1111. Delivery dates shall be IAW the milestone schedule cited in paragraph 5.0 of the SOW. The following FAR PROVISIONS and CLAUSES apply to this solicitation and are hereby incorporated by reference: Provision 52.212-1, Instructions to Offerors-Commercial Items (request Atch 7, Addendum, for technical proposal requirements). Provision 52.212-2, Evaluation, Commercial Items (fill-ins: Technical/Management, Past Performance and Price. (See also Atch 7). Offerors shall include a completed copy of the provision FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. Offerors shall also include a completed copy of the provision DFAR 252.212-7000, Offeror Representations and Certifications, Commercial Items, with its offer. A full-text copy of the representations and certifications appears on the Internet. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items; FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. Specifically, the following provisions are applicable: FAR 52.203-6, 52.203-10, 52.219-8, 52.219-14, 52.222-26, 52.222-35, 52.222-36, 52.222-37. Additional FAR clauses that are applicable are 52.217-8, Option to Extend Services; 52.217-9, Option to Extend the Term of the Contract (fill-ins (a) within 15 days; (c) 36 months); 52.232-18, Availability of Funds and 52.232-33, Mandatory Information for Electronic Funds Transfer Payment. The following DFAR provisions and clauses apply to this solicitation and are incorporated by reference: 252.212-7001, Contract Terms and Conditions Required to Implement Statutes Applicable to Defense Acquisitions of Commercial Items, 252.204-7004 -- Required Central Contractor Registration and 252.232-7009 -- Payment by Electronic Funds Transfer (CCR). BASIS FOR AWARD: The Government will award one contract resulting from this solicitation to the responsible offeror whose proposal conforms to the solicitation, who has an acceptable rating on the technical/management capability factor, and who is determined "Best Value" to the Government considering past performance and price. The Government may conduct a tradeoff between past performance and price and reserves the right to award to other than the low priced offeror. It is anticipated that award will be made without holding negotiations or discussions. Offerors can submit written questions about this combined synopsis/solicitation to Ms Sharon Hinds at the address cited at the beginning of this notice (telefax is acceptable at 202-767-7814) NLT 24 Aug 98. The proposed technical approach should meet all requirements of the SOW. Offers shall provide proposed costs IAW Atch 2 of this solicitation. Technical/management capability will be rated acceptable or unacceptable. The following factors shall be used to evaluate offers in descending order of importance: 1) Technical/Management capability; 2) Past Performance and 3) Price. The offeror shall describe its past performance on all directly related or similar contracts held which are of similar scope, magnitude and complexity to this requirement. The Government will consider Termination for Default, delinquencies, failure to comply with SOW requirements, cost overruns, amount of rework and any other information which reflects the overall quality of the offeror's past performance. The offeror shall provide the following information regarding past performance (if some do not apply, annotate such): contract number(s), job title, POC name and telephone number at the federal, state or local Government or commercial entity for which the contract was performed, dollar value of the contract, detailed description of the work performed, names of subcontractor(s) and number, type and severity of any quality, delivery or cost problems in performing the contract, the corrective action taken and the effectiveness of the corrective action. The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources including sources outside the Govt. Offerors lacking relevant past performance history will receive a neutral rating for past performance. The offeror must provide information requested above for evaluation or affirmatively state that it possesses no relevant directly related or similar past performance. An offeror failing to provide the past performance or to assert that it has no relevant directly related or similar past performance will be considered ineligible for award. Responses to this solictation are due by 2:00 P.M. EST on 28 Aug 98. The proposal shall be signed by an official authorized to bind the company. Offerors shall submit 4 copies of their proposal. It shall be broken down by Volume I, consisting of the SF 1449 (see Atch 1), Price Proposal-Schedule B (see Atch 2), and Representations and Certifications IAW FAR 52.212-3 and DFARS 252.212-7000; and Volume II which will consist of the Technical Proposal (limit 25 typewritten, double-spaced pages -not including two samples and vita/resume) and Past Performance Data, bound separately.
