The Stratton VA Medical Center, Albany, NY is seeking to contract with local hospitals, ambulatory care facilities, group practices or clinics who have the qualification, experience and expertise in providing quality mental health care services to veteran beneficiaries residing in specified communities within the Facility's Primary Service area. Those communities are: Sidney, NY, Elizabethtown, NY, Malone, NY and Plattsburgh, NY. The Basic Provider will provide specified mental health care services in a private hospital or clinic environment. Mental Health Care includes longitudinal outpatient mental health care for the purpose of prevention and detection of mental illness and subsequent management and treatment of mental health conditions in eligible veterans. The care will be provided by mental health providers who may be psychiatrists, clinical social workers, psychologists, psychiatric clinical nurse specialists. The care provided should be continuous, accessible, and coordinated. Proposals will besolicited for a Base Year and four (4) yearly renewal options.
