This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F0470098T2004. This solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-5. The standard industrial classification code is 8071. The size standard is $5.0M. This acquisition is unrestricted; it is not set aside for small businesses. The acquisition is for the following services: Line Item 0001, Contractor shall provide the following service: blood tests and other related genetic prenatal screening, prenatal alpha fetoprotein screening to include analysis for Human Chorionic Gonadotropin (HCG), Unconjugated Estriol (UE3) and Alpha-Fetoprotein (AFP) on approximately 20 samples per month; the Government will perform the blood draw and samples will be either mailed or transported by courier to contractor's facility by the Government; test results shall be mailed to the Edwards AFB hospital by and at the expense of the contractor; "screen positive" patients shall receive diagnostic services at a prenatal diagnostic center, at a distance of no more than 150 miles from Edwards AFB, CA 93524 and shall include: professional genetic counseling, ultrasound examination, amniocentesis and analysis of amniotic fluid.
