310/363-5088 Description: This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
contractor shall provide a qualified registered nurse, herein referred
to as the Integrated Care Manager (ICM). The ICM shall be responsible
for providing disease and case management to Government beneficiaries.
Care by the ICM will be provided at the medical treatment facility
(MTF). The ICM will evaluate proactive triggers to identify potential
clients suitable for effective disease/case management intervention.
The ICM shall identify opportunities for intervention which affect the
individual and/or population at risk. The ICM will seek authorization
or consent for case management from recipient of services. The ICM
shall conduct a thorough and objective evaluation of the client's
current status covering such elements as: rationale for entry into the
system, physical, psychosocial, environmental, spiritual, financial,
vocational, primary care manager, learning capabilities/self-care,
health status expectation, and community reintegration potential. The
ICM shall assess resource utilization and cost management; the
diagnosis, past and present treatment course and services; prognosis,
goals, treatment, and provider options. The ICM shall demonstrate
ability to interview, research, understand client's diagnosis,
prognosis, care needs, and outcome goals of treatment plan, understand
cost-containment strategies, and critically analyze situations
identifying barriers to goal attainment. The ICM shall have knowledge
in: health, social service, and funding sources, legal scope of
practice of healthcare providers, legal requirements for reporting
abuse, laws and procedures regarding client confidentiality, the
American Disabilities Act, Workers' Compensation laws and act in
accordance with them. The ICM shall also have knowledge of computer
software applications to include spreadsheets, word processing,
presentation software, electronic mail, and internet access. The ICM
shall maintain communication with client and family to convey
information on healthcare services, as well as with treatment team and
providers to objectively discuss the care plan. Outcome goals for ICM
are as follows: identify case management goals focusing on meeting
client's needs, plan with client/family a goal-oriented care process
that analyzes and gives direction to a treatment plan, report
quantifiable quality care and/or life improvements, and recommend
referral sources. The ICM shall be guided by ethical principles and
provide service with respect for autonomy, dignity, privacy and rights
of the individual. The ICM shall be responsible for creating and
maintaining medical entries. Patient lists shall be treated as
privileged information. The ICM shall only release medical information
obtained during his/her contract to other staff involved in client
care. He/she must be available Tuesdays and Thursdays from 7A.M. to
4P.M. September 1, 1998 through August 30, 1999, excluding federal
holidays. The ICM will have the option of continuing this service for
two additional service following August 30, 1999. The ICM is required
to begin full performance of the contract requirements within 60 days
after contract award. The government will conduct a criminal background
check. The ICM must possess the following to be considered: copy of
valid, current unrestricted state nursing license, and applicable
national Certification, training in disease, case, and utilization
management, Current Basic Life Support (BLS) certification, a signed
consent for release of information, certification of current physical
examination (must be current as of 12 months prior to application). A
Masters degree in nursing or other related discipline is not required,
but desired. The ICM shall have a background to include no less than
5 years in medical management positions. In addition, a sworn affidavit
of truthfulness is required indicating experience, training, and
technical expertise in the type of care to be rendered. All responsible
sources may submit a bid, proposal, or quotation which shall be
