310/363-5088 Description -- This is a combined synopsis/solicitation
for commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. The announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
contractor shall provide a qualified registered nurse, herein referred
to as the Nurse Health Educator (NHE) to provide Community Health
Education services to Government beneficiaries. The contractor will
provide care at the medical treatment facility (MTF) and specifically
with the Health and Wellness Center (HAWC). The NHE will assess, plan,
direct, implement, evaluate, and coordinate health and wellness
education. The NHE shall perform procedures compatible with the HAWC's
operating capacity and equipment. New procedures/services shall not be
introduced without prior recommendation to, and approval of, the HAWC
Director. NHE duties will include: coordinate with HAWC Director to
develop health education budget, develop continuity files, research,
develop, acquire, maintain, and implement necessary instructional
material needed to provide an effective health education program, and
coordinate activities with the Chief of Health Care Integration, the
Prevention Committee Chair, and/or HAWC Director to ensure clients are
best served. The NHE shall be involved in the following: provide
outpatient prevention health care, conduct and plan health and wellness
education classes in conjunction with other health promotion and
disease prevention experts, provide health and wellness education to
individuals on subjects involving tobacco use cessation, hypertension,
safety, managing chronic diseases, communicable diseases, stress
management, exercise, nutrition, healthy lifestyles and intentional and
unintentional injuries. The NHE will work with HAWC staff in promotion
of national health themes, attend meetings as directed by the HAWC
Director, and the NHE will actively market health and wellness
programs. Client satisfaction, outcomes/behavior changes will routinely
be evaluated. Quantifiable impact, quality of care/life improvements
shall be reported by the NHE. Documentation must be prepared to
meet/exceed established standards of the HAWC. The NHE shall refer
clients to MTF providers and registered dietitians when clients show
signs of needing medical intervention, as well as refer clients to
other base activities. The NHE will be responsible for creating,
maintaining, and disposing of records. Client lists must be treated as
privileged information. Proficiency in windows based applications is
mandatory. The NHE shall respect the basic rights of clients. Only
medical information obtained during the course of this contract shall
be released and only to MTF staff. The CO shall require the Contractor
or his/her representative to meet with the CO, contract administrator,
QAE, and other government personnel at least quarterly. The NHE must be
available to work Monday, Wednesday, and Friday from 7A.M. to 4P.M.
September 1, 1998 through August 30, 1999, excluding federal holidays.
The NHE will have the option of continuing the service for two
additional years following August 30, 1999. The NHE is required to
begin full performance of the contract requirements within 60 days
after contract award. The government will conduct a criminal background
check. The NHE must possess the following to be considered: copy of
valid, current state nursing license from the state where HAWC is
located, copy of certification as Certified Health Educator Specialist
(CHES), Current Basic Life Support (BLS) certification, signed consent
for release of information, and certification of current physical
examination (must be current as of 12 months prior to application). A
bachelor's degree in nursing, community health, adult education, health
sciences, or a related field from an accredited university, or a
master's degree in health education, health promotion, or public health
with specialization in health education is necessary. The NHE must have
a minimum of 2 years experience within the last 5 years in the health
education area. In addition, a sworn affidavit of truthfulness is
required indicating experience, training, and technical expertise in
the type of care to be rendered. All responsible sources may submit a
bid, proposal, or quotation which shall be considered by the agency.
