Transamerica Occidental Life Insurance Company (TOLIC), which currently is under contract with HCFA for Medicare Part, B claims processing, is seeking sources for printing an annual supply of Medicare Envelopes. Planned specification and volumes are anticipated as follows. Estimated volume, for 12 months supply is 10,000,000. I TECHNICAL REQUIREMENTS: (1a) Two separate envelopes stocks are required, (b) Both to run on Pitney Bowes 8 series inserters. (c) Envelope #1 -- 2,500,000 envelopes to be printed with Glassine window material but only 750,000 at one time. (d) Overall size #10: 14-1/2 x 9-1/2 weight 24#, color white, wove stock, black ink. (e) Glassine window size: 1-1/2 x 4-1/4. (f) Window Location: 1` from left edge and 3/4` from lower edge. (g) Glue through across top of window as close to its edge as possible. (h) Flap size: 1-3/4. (i) Inside tint gray OK to flexo. (j) Envelope construction: Side seams (inside), seam must be 3/4` wide. (k) The vertical gluing on seams must be centered on seams. (l) Double scored envelope and 1/4` throat dimension. (m) Packing to be 2,000 per box. (n) Bi-monthly deliveries of 250,000 required. (2a) Envelope #2 -- 8,000,000 envelopes to be printed with Glassine window material but only 1,600,000 at one time. (b) Overall size #10: 4-1/2" x 9-1/2", weight 24#, color white, ink black, wove stock black ink. (c) Glassine window size: 4-1/4" x 1-1/2". (d) Window location: 1" from left edge and 1/2" from lower edge. (e) Glue through across top of window as close to its edge as possible. (f) Flap size: 1-3/4". (g) Inside tint gray OK to flexo. (h) Envelope construction: Side seams (inside), seam must be 3/4" wide. (i) The vertical gluing of seams must be centered on seam. (j) Double scored envelope and 1/4" throat dimension. (k) Packing to be 2,000 per box. (l) Bi-weekly deliveries of 400,000 required. (m) The envelope construction must be acceptable to the Pitney Bowes Model 8-Series Inserter. Otherwise envelopes must be re-done at Vendor expense. (n) OK to run on WEB Press with the above specifications. OTHER REQUIREMENTS: All interested vendors must submit their responses to include the following: (1a) unit, (1b) transportation & packing charge per carton, (2) 5 (five) client references, (3) statement of previous experience, (4) SADBUS/MBE qualification statement, (6) TOLIC onsite testing ability stmt. (7) qualification statements stating the vendor's ability to meet the technical specifications above (I- 1a-2n), (8) responsiveness statement for meeting the Other Requirements above (II 1-6) Vendors bids will be evaluated 70% on cost and 30% on providing and meeting the requested information in technical and other requirements specified in I & II above.
