commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotes are
being requested and a written solicitation will not be issued.
Solicitation 502-28-98 is a request for proposal. The solicitation
document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 97-06. The SIC code is 3679 and
the small business size standard is 500. VA Medical Center
requirements: 1. APC 300/ICC 200 Argon Plasma Coagulator, which
includes: (1) Product #10128-205 ICC 200 Electrosurgical
Generator/Endo/Argon (2) Product #10132-011 ICC 300 Argon Plasma
Coagulator; (3) Product #1063-2 Argon Gas Cylinder Tank/Full (4)
Product #20132-065 Pressure Hose w/reduction valve ICC/APC (5) Product
#20185-009 Mounting Cart (6) Product #20189-027 Double Pedal
Footswitch, (7) Product #20189-022 Footswitch Connector(8) 20192-082
Connecting Cable to ICC-200/APC-300, (9) Product #20132-046 reusable
probe connector hose, (10) Product #20193-044 nessy monitoring pads,
(11) Product #20183-010 Bicap adapter and ignition tester for probes.
This equipment is to help control G.I. Bleeding without exposing the
patient to the hazards of laser. Equipment should be installed in
accordance with manufacturers specifications and have a standard
warranty. Suggested source of this equipment is Electrosurgical

800-778-3723. Offerors responding to this announcement shall provide
the information contained in FAR clause 52.212-1, Instructions to
Offeror-Commercial Items, and shall include a completed copy of
provision 52.212-3, Offeror Representations and
certification-Commercial Items. An authorized representative must sign
the representations and certifications. The offeror shall also provide
its Contractor Identification Number (DUNS), and Tax Identification
Number (TIN). Clause 52.212-2, Evaluation-Commercial Items. The
Government will award a contract resulting from This solicitation to
the responsible offeror whose quote will be most advantageous to the
Government, price and other factors considered. Evaluation factors are
1). Price; 2) Technical capability (equal importance). Offer shall
submit three references that you have performed a similar or same type
service. Clause 52.212-4, Contract Terms and Conditions-Commercial
Items. Clause 52.212-5, Contract Terms and Conditions required to
implement Statutes or Executive Orders-Commercial Items. Clause
52.228-5, Insurance-Work on a Government Installation. Quotes are due
