combined synopsis/solicitation for commercial item prepared in
accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotations are being requested and
no written solicitation will be issued. This is 100 percent set aside
for Small Business. See Numbered Note 1. SIC Code is 8744. Size
standard is 500. This solicitation identified as SOL NO. R98-10 is
issued as a Request for Quote. This solicitation and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-06. Quotes shall be in accordance with all requirements of
this notice including those which are included by reference. This
requirement consists of a soil remediation project, including
investigation and cleanup. Project to be conducted in compliance with
Washington Administration Code 173-340, Model Toxics Control Act --
Cleanup (MTCA). The project will be performed under the
VoluntaryCleanup Program with Department of Ecology (DOE) oversight and
approval assuring that the requirements of MTCA have been met. All DOE
costs will be included in the contract. The project includes
excavation and proper disposal of approximately 50 cubic yards of
petroleum contaminated soil associated with release from a fuel storage
shed. The shed has been removed. The shed's concrete pad and drain pipe
is still in place. The pad and drain will be considered a part of the
disposal project. The site is located within a road maintenance
facility. The facility is on a parcel of land leased and operated by
the Bureau of Indian Affairs Olympic Peninsula Agency. The land is
located on the Quinault Indian Reservation in the state of Washington.
The legal location is the SW, SW, Section 29, Township 22 N., R10 W,
Willamette Meridian. The general geology of the area is alluvial,
consisting of mostly of silt, sand, and gravels. Depth to groundwater
is not known. No storm drains or other conduits for easy offsite
migration of contamination exist. The nearest surface water is Cook
Creek, about one quarter mile west. The nearest public facility is the
Quinault Hatchery about one half mile west on Cook Creek. The site is
located in a rural area, with sparse population. In 1994 two
underground storage tanks (UST) and 30 cubic yards of soil contaminated
with petroleum hydrocarbons were removed adjacent to the present
remedial site. The soils were tested, and treated but not re introduced
to the pit. The shed has since been removed. The present project is an
extension of the original soil investigation and cleanup. The testing
of soils and analytical reporting will be performed by a certified
site assessor registered with the International Fire Code Institute
(IFCI). Copies of the certification will be made available to the BIA
upon request. The contractor shall have an individual(s) registered
with DOE to perform site assessments and sign the Site Assessment
checklists as provided by DOE. The registered site assessor shall be
present on the site at all times sampling is performed, and shall be
responsible for the collection of data and information necessary to
complete site assessments as required by DOE and described in the
contract documents. The contractor is requested to furnish a per unit
price for the following items: 1. The contractor shall include a unit
cost per cubic yard of transportation and disposal of contaminated
soils. Contractor shall perform the work in compliance with federal,
state, and local transportation and disposal regulations. Copies of
disposal receipts, name, location, and description of fate of
contaminated soils shall be provided to the BIA within 30 days of final
disposal. 2. Per hour rates for project site assessor, technicians,
reviewers, and any other personnel required for the work. 3.
Mobilization and demobilization costs for the project. 4. The
contractor shall prepare a site assessment report and deliver a copy to
the BIA within 60 days from the beginning of soil excavation. The site
assessmentreport shall include the name of the person registered with
the DOE who conducted the site assessment; location of the site on a
vicinity map; summary of background information; summary of field
sampling activities, including the number of and types of samples that
were collected, the methods used to collect and analyze the samples
and the name and address of the laboratory used to perform the
analyses; a site sketch showing the location and unique identifying
number for all field samples collected for laboratory analysis; a
description of any factors that may have compromised the quality of the
data or validity of the results; a table showing, for each field sample
collected, laboratory results for all indicator constituents analyzed;
a copy of the original laboratory report; and a copy of the Site
Assessment. A lump sum price for disposal of concrete slab and drain
pipe. Prospective contractors are invited to contact the Olympic
Peninsula Agency of the Bureau of Indian Affairs for access to files
concerning the previous site cleanup. The contact person at the Agency
