synopsis/solicitation for a commercial item prepared in accordance
with the format in Far subpart 12.6 as supplemented with additional
information included in this notice. Written Quotes are being requested
and a written solicitation will not be issued .The reference number for
this procurement is RFQ 500-69-98 and incorporates the clauses and
provisions that are in effect through Federal Acquisition Circuler
97-06. The Standard Industrial Classification Code is 3841. Quote
should be for the following item: Five (5) left hand and Four (4) right
hand Modular Toilet for a Total of Nine (9) each Modular Toilet, Type
304 Stainless Steel. Cabinet Frame with cabinet doors constructed of
high density woodcore finished with high pressure laminate. With
hydraulic flush valve, toilet tissue holder and hinged side panels.
Countertop finished with high pressure laminate. Stainless Steel
lavatory 9 1/2" x 14 1/2" x 6" deep. Stainless steel elongated toilet
with hinged seat. Toilet shall be flooroutlet and withstand a load of
5,000 pounds without damage. White enviro-glaze on sink bowl and
toilet. Backsplash on back wall; foot pedal valves. Quoter is also
required to provide installation instructions. Contractor shall include
warranty information. Quotes received shall specify FOB DESTINATION.
The provisions at FAR 52.212-1 Instructions to Offerors-Commerical
Items (AUG 1998) applies to this acquisition. FAR 52.212-2 Offerors
Representations and Certifications applies to this acquisition and a
completed signed copy of this provision shall be submitted with the
quote. The Representations and Certifications may be obtained vai the
internet at: http://www.arnet.gov/references/references.html (FAR) .
The clause at FAR 52.212-4 Contract Terms and Conditions (APR 1998)
applies to this acquisition. The clause at FAR 52.212-5 Contract Terms
and Conditions Required to Implement Statues or Executive Orders
-Commerical Items (JUN 1998) applies to this acquisition; under para.
(b) the following clauses are incorporated FAR 52.222-26 Equal
Opportunity, FAR 52.222-35 Affirmative Action for Disabled Veterans and
Veterans of the VietNam Era (APR 1998), FAR 52.222-36 Affirmative
Action for Workers with Disabilities, 52.225-3 Buy American Supplies
(JAN 1994). Techincal/Descriptive Literature shall include but not be
limited to technical specifications, product picture, product features,
etc, and shall be submitted with the quote or the quote will be
rejected as non-responsive. Technical acceptability will be determined
by information submitted by the quoter providing a description in
sufficient detail to show that the product meets the governments
requirements. The following factors, in descending order of importance,
will be used to evaulate quotes: (1) determination of product
acceptability, (2) lowest priced quotations, (3) delivery schedule and
(4) technical ability. Quotations are due by Close of Business,
5:00PM, September 21, 1998. Anything received after that date and time
