This is a combined synopsis/solicitation for commercial items prepared
in accordance with format in FAR Subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation and a written solicitation will not
be issued. (ii) The solicitation number F11623-98-T2013 is being issued
as a Request for Quotation (RFQ). To request a copy of the Statement of
work, contact John Traylor, in writing, 201 E. Winters, Scott AFB IL
62225-5015. FAX: (618)256-5237. (iii) Provisions and clauses are those
in effect through Federal Acquisition Circular 97-04. (iv) This
solicitation is reserved as a small business set-aside and the
associated standard industrial classification code is 5531 and the
small business size standard is 750 employees. (v) There is one job.
Contractor to furnish labor, equipment, materials, supplies,
transportation, required to convert air conditioning unit in bldg 1575
from R-11 to R123 according to the attached statement of work. (vii)
Delivery of equipment required 7 days after award to Bldg. 1575.
Acceptance of equipment and performance start shall be coordinated with
Mr. Macke. FOB shall be Destination. Site visit covering task in
Statement of work will be conducted in Bldg 1575. (viii) The provision
at FAR 52.212-1, Instructions to Offerors Commercial, applies to this
acquisition. Subparagraph (a)(7) is changed to read: any resulting
contract will be subject to FAR 52.232-33, Mandatory Information for
Electronic Funds Transfer Payment. (ix) The provision at 52.212-2,
Evaluation Commercial Items, applies to this acquisition. Paragraph (a)
is supplemented to read: lowest price, technically acceptable are the
evaluation factors. Submission of quotes should include technical
information and specification data. The Government will award a
contract resulting from this solicitation to the responsible offeror
whose offer conforming to the solicitation will be most advantageous to
the Government, prices and other factors considered. The following
factors shall be used to evaluate offers: (listed in descending order):
technical capability of the offeror to meet the Government
requirement/past performance; price. The offeror shall provide a list
of at minimum 5 of the most recent and relevant past and present
contracts performed for Federal agencies and commercial customers with
in the last 2 years. Furnish the following information for each
contract: a) Company/Division Name; b) Product/Service; c) Contracting
agency; d) Contract Number;e) Contract Dollar Value; F) Period of
performance; g) Name, Address, fax number, and telephone number of the
Contracting Officer; h) Comments regarding compliance with contract
terms and conditions; i) Comments regarding any known performance
deemed not acceptable to the customer or not in accordance with the
contract terms and conditions ; j) Evaluations are based on "go no go"
basis. Offeror meets technical capability by submitting offer. Any
other information the offeror deems relevant.(x) Offerors must include
a completed copy of the provision at FAR 52.212-3, Offeror
Representations and Certification Commercial Items, with quote. Copy of
provision may be requested by fax (number listed above). Attn: John
Traylor. Quote will not be considered without completed provision. (xi)
The clause at FAR 52.212-4, Contract Terms and Conditions Commercial
Items, applies to this acquisition. Paragraph (o) is supplemented to
include the following: Contractor shall provide the government with all
standard commercial warranties. (xii) The clause at FAR 52.212-5,
Contract Terms and Conditions Required to Implement Statues or
Executive Orders Commercial Items, applies to this acquisition. The
clause at the following subparagraphs apply (b)(7), (b)(8), (c)(1) and
(c)(5). The clause at DFARS 252.212-7000, Offeror Representations and
Certifications Commercial Items, and DFARS 252.212-7001, Contract
Terms and Conditions required to implement Statues or Executive Orders
applicable to Defense Acquisitions of Commercial Items apply to this
acquisition. DFAR 252.225-7001 applies. (xiv) The Defense Priorities
and Allocations System (DPAS) does not apply to this acquisition. (xv)
The Commerce Business Daily numbered Note 1, 100% Small Business
Set-aside. Contractors are required to be registered on the Central
