Q -- HOME MONITORING SERVICES FOR CHRONIC HEART FAILURE PATIENTS SOL RFQ 662-76-98 DUE 100998 POC Annie Bell, Contracting Officer, (415) 750-2240 The VA Medical Center, San Francisco, requires Home Monitoring Services to include patient education program covering; 1) the causes, signs, symptons, and outcome of chronic health failure (CHF); 2) issues related to medications prescribed for CHF; and 3) procedures for improving compliance with physician recommendations, tracking of measurements and procedures. The contractor shall provide registered nurses with a BSN, MSN, and experience working with heart failure patients. The contractor's nurse(s) shall provide weekly telephone contact with the patient and answer patient initiated telephone reports of altered symptons and vital signs. The nurse(s) will provide home monitoring services Monday through Friday, 8:00a.m. to 5:00 p.m. (40 hours per week).
