synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with the additional
information included in this notice. This announcement constitutes the
only solicitation; quotes are being requested and a written
solicitation will not be issued. This solicitation, F14614 98 Q5079, is
being issued as a Request for Quotation (RFQ). The solicitation
document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 97-5. This is a 100% small
business set-aside. The Standard Industrial Classification is 8011,
with a small business being not more than $5 million average annual
gross revenue. There are three Contract Line Items (CLINS). The
contractor shall provide all labor and transportation necessary to
provide ultrasonography service as stated in the Statement of Wok
(SOW), except as otherwise specified in the SOW as government furnished
property and services at Bldg 250 (22d Medical Group),McConnell AFB,
KS. CLIN 0001: For the basic period of 01 Oct 98 30 Sep 99; CLIN 0002:
For the first option year the period 01 Oct 99 30 Sep 00; CLIN 0003:
For the second option year the period 01 Oct 00 30 Sep 01, The SOW is
as follows: Contractor shall: A. Provide an ultrasonographer to perform
procedures associated with ultrasonography and procedures requested by
the radiology department for the physicians at Bldg 250, 22d Medical
Group. B. Scheduling for procedures will be done by 22d Medical Group
personnel who will attempt to consolidate all ultrasound appointments.
The ultrasonographer will be required at the Medical Treatment
Facility for two days a week from 7:30 am to 4:30 pm. Usually these
days will be Wednesday and Friday, but is negotiable for purposes of
procedures or scheduling needs. Additional time may be scheduled for
examinations as needed and determined by the requirements of the
physicians assigned to the 22d Medical Group. C. Paperwork and patient
registration will be performed by medical group personnel. Studies
performed will be the property of the 22d Medical Group. D. The
ultrasonographer is required to conform to all Joint Commission on
Accreditation of Health Care Organizations and American Medical and
American Registry of Radiological Technologists standards. E.
Equipment, film and supplies will be provided by the 22d Medical Group.
F. Payment will be on a fee for service schedule utilizing CPT codes
and will be for the technical portion of the examination only, unless
otherwise specified (TC=Technical Component, PC=Professional Component,
T=Technical). The only procedure containing both technical and
professional fees will be the echocardiography studies. Diagnostic

acquisition. The Government intends to evaluate offers and award a
contract without discussions; therefore, qualified contractors should
submit a quotation with the best possible price. Provisions at FAR
52.212-2. "Evaluation Commercial Items," apply to this order. Lowest
overall price shall be used to evaluate offers. Offerors shall include
a complete copy of the provision at FAR 52.212-3, "Offeror
