66 -- THERMAL POLYRAPH INSTRUMENTS SOL FBICU9838 DUE 092598 POC Sharon K. Starkey, Contract Specialist This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) FBICU9838, the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 97-05. This solicitation is under the Simplified Acquisition Procedures (SAP) in FAR Subpart 13.6 Test Program. This solicitation is 100% Small Business Set-Aside. The Standard Industrial Classification (SIC) Code is 3829, and the Business Size Standard is 500 employees. This will be a Fixed Price Indefinite Delivery-Indefinite Quantity Contract (IDIQ). This IDIQ Contract will be for a base year of twelve (12)months with four (4)one-year option renewals. The Federal Bureau of Investigation (FBI) has a requirement to purchase 25 thermal polygraph instruments. The estimated minimum order amount for the contract is 25 units and the estimated maximum, over the life of the contract is 200 units. The thermal polygraph instrument shall have the following performance specifications: thermal recording of charts, two multi-function channels, one all purpose channel, calibration (in case), tool kit and include the following attachments: (2) Pneumo Assembly, (1) Cardio Cuff and Pump Bulb Assembly and GSR Finger Electrodes. The FBI have 25 used Lafayette polygraph instruments that they will entertain as trade-ins. Warranty: in accordance with FAR 12.404 (b) offerors are required to offer the Government at least the same warranty terms, including offerors of extended warranties offered to the general public in customary commercial practice; FOB: Destination to FBI, Cheverly Industrial Park, Cheverly, MD. Delivery shall begin no later than thirty (30) days after receipt of order. The following clauses are incorporated by reference and apply to this acquisition: FAR 52.212-4 Contract Terms and Conditions-Commercial Items (Aug 1996); and FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items (Aug 1996): Paragraph (a)(1)(2) and paragraph (b)(1) through(17)is appropriate. FAR provision 52.212-2 Evaluation -- Commercial Items (October 1995) with Addendum to include in paragraph (a) the Government will award a contract resulting from this solicitation which will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability; (2) Past Performance; and (3) Price; in descending order of importance. Technical capability and past performance, when combined are significantly more important than price. For evaluation of past performance, offerors are required to submit information of the last three (3) government/commercial contracts for thermal polygraph instruments including the name of Company, date awarded, award amount, contract number, company/government's point of contact with address and telephone number. Addendum to include in Paragraph (b) Options, the FBI will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options shall not obligate the Government to exercise the option(s). Each offeror shall include a completed copy of the FAR provisions at 52.212-3, Offeror Representation and Certifications (June 1996) with its offer. The following additional clauses apply to this acquisition: FAR 52.217.9 Option To Extend Term of the Contract (Mar 1989)with Addendum to include in paragraph (a) the Government may extend the term of this contract by written notice to the Contractor within (insert) thirty (30) days and include in paragraph (c) the total duration of this contract, including the exercise of any options under this contract, shall not exceed (insert) sixty (60) months; and FAR 52-216-22 Indefinite Quantity -- with Addendum to include in paragraph (d)the contractor shall not be required to make any deliveries under this contract after (insert) sixty (60) months.
