Holloway, Contracting Officer (970) 242-0731 X2037 This is a combined synopsis/solicitation for commercial items/services prepared in accordance with format in subparts 12.6 and 13.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation to be issued, proposals are being requested from this announcement, a written solicitation will not be issued. The Request for Proposal number is 575-471-99. This solicitation incorporates provisions and clauses in effect through FAC 97-02. Standard Industrialization Code (SIC) of 8999 is applied. This solicitation is issued subject to the availability of government funds at time of award. This solicitation is unrestricted for the following service: Contractor to provide installation of a DS1 Circuit to provide connectivity between two stations. This will allow voice/data to be transmitted over the same line. The stations involved are the Department of Veterans Affairs, Medical Center in Grand Junction, CO and the Community Based Outpatient Clinic in Montrose, CO. Phone service in these areas is provided by U S West, therefore, the contractor should be a US West authorized dealer. All responsible sources may submit a proposal which shall be considered by the agency. The following provisions and clauses are applicable and are hereby incorporated into the solicitation by reference and will be provided by the contracting officer in full text upon request: FAR 52.212-1, Instruction to Offerors Commercial Item applies. Addendum clauses FAR 52.215-18 Facsimile Proposals also applies. Offerors are required to complete and include a copy of the following provisions with their proposals: FAR 52.212-3, Offeror Representations and Certifications Commercial Items, FAR 52.212-5 Affirmative Action for Special Disabled and Vietnam Era Veterans; FAR 52.222-26, Affirmative Action for Handicapped Workers: FAR 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era. Award will be made based on past performance, qualifications of the candidate, and the ability to meet the time frame required to have the clinic operational approximately November 1, 1998.
