65 -- SOLE SOURCE FOR PURCHASE OF URETERORENOSCOPE SOL 69D-426-98 DUE 092598 POC Diane Brazale, Contracting Officer (414) 902-5403 The Great Lakes Healthcare System Acquisition Center intends to negotiate a sole source contract with Richard Wolf Medical, Vernon Hills, IL for the purchase, of two (2) flexible ureterorenoscope for the VA Medical Center, Westside Chicago IL. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Far Subpart 12.6 as supplemented with additional information included in this notice. SIC Code 3845. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-05. This procurement shall include all costs associated with the assembly, delivery, installation, user's manuals and on-site training/PM instructions as required by the hospital. The Government believes that Richard Wolf Medical is the only source that can provide a complete system that will satisfy the agency requirements. This procurement shallinclude: Two (2) Flexible Koch Ureterorenoscopes (Part No. D7325.172), 7.5 FR with 3.6 capacity; Two (2) Fiber Light Cables and Two (2) Xenon Light Source (Part No. A5131.001) with multiport face, 300 Watt (interchangeable and compatible with existing equipment). Payment will be made in arrears upon complete installation, training and final acceptance of the equipment by the Government. This will be a firm fixed price contract. All Equipment shall be shipped FOB DESTINATION to the VA Medical Center, Lakeside Division, 333 East Huron Street, Chicago IL 60611. Year 2000 compliance means that information technology accurately processes date/time data (including, but not limited to, calculating, comparing, and sequencing) from, into, and between the twentieth and twenty-first centuries, and the years 1999 and 2000 and leap year calculations. The offeror is required, as part of the offer, to provide documentary evidence that the equipment, replacement parts, components, hardware, software and firmware offered is year 2000 compliant.
