Acquisition Center intends to award a contract for purchase of one (1)
each, Topcon TRG-50EX Digital Retinal Camera and the IMAGEnet Digital
Capture System, OR EQUAL, for the Iron Mountain MI VA Medical Center.
This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in Far Subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation. Quotes are being requested under RFQ
69D-429-98 and a written solicitation will not be issued. Solicitation
documents and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 97-04. SIC (3841). Contractor
shall provide the following Brand Name Items, OR EQUAL: ITEM #1 -- One
(1) each, Topcon TRG-50EX Retinal Camera, with the following features:
(a) AIT-11 Table with Double table top. (b) 1MT-10 Camera Back; (c) No
Filter Sets. ITEM #2 IMAGEnet Digital imaging system (Capture station,
Fileserver/review station); including the following features: A. One
(1) each, 800 x 600 FA/Color Capture System, including the following
features: (a) Pentium II 333 MHz MMX Based Desk Top PC; (b) (2) 4.1 GIG
HD 64 MB RAM + 4 Mbyte Controller Card; (c) Modem (56K Baud); (d) CD
Archiving Capability to interface to Retinal Camera; (e) Battery Backup
and Line conditioner; (f) System table and necessary wiring; (g) Color
Image Capture/Display Card; (h) 17" Color UVGA Monitor; (i)
Communication and Utility Software; (j) CD Archiving 4X Write-6X Read;
(k) CD Reader 24X; (l) Interface to Retinal Camera (50 VT, 50X or
50EX); (m) Sony PAL 950P Color 3 -- Chip CCD Camera; (n) Battery Backup
and Line Conditioner; (o) System Table and necessary wiring. B.
Networked Laser Review Station@Laser Room, including the following
features: (a) Pentium II 333 MHz MMX Based Desktop PC; (b) 2.56 GIG HD
+ 32 MB or greater; (c) 3.5" Disk Drive; (d) CD Reader 24X; (e)
Network Card and cabling -- up to 150 ft; (f) 17" Color SVGA Monitor;
(g) IMAGEnet Review Station for Windows. C. Color Printer (Epson Stylus
800 -- Ink Jet Color/B&W Printer); D. Network capabilities: RGB or S
Video Output for transmission of imaging for Telemedicine purposes;
Network Compatibility with Ethernet LAN & TCP/IP Protocol; Queryable
Database by standard query methods via the LAN network. Client
developed software by user can access various Database objects for
information retrieval and/or for the purpose of updating Database
tables. E. Manuals: Provision of User and Technical manuals and
provision of database programming interface information. i.e.,
procedures, functions, table structure information, connectivity
protocols, rules etc. The VA intends to use this equipment both with,
and independent of, the V-Tel F.R.E.D. (Friendly Roll-about Engineered
for Doctors) Telemedicine System currently in place at the Iron
Mountain VA Medical Center. All equipment, replacement parts,
components, hardware, software and firmware containing information
technology delivered under this contract shall be Year 2000 Compliant.
Year 2000 Compliance means that information technology accurately
processes date/time data (including, but not limited to, calculating,
comparing, and sequencing) from, into, and between the twentieth and
twenty-first centuries, and the years 1999 and 2000 and leap year
calculations. The purchase order will be issued to the vendor whose
quote is most advantageous to the VA, considering price and other
factors. Vendors must provide, with their quotes, information (e.g.,
commercial literature) describing their equipment and demonstrating
that their equipment meets all the requirements and is EQUAL TO the
items specified above. This information must be provided for both the
Brand Name items specified above and any [OR] EQUAL items that are
being quoted. The offeror is also required, as part of the offer, to
provide documentary evidence that equipment, replacement parts,
components, hardware, software and firmware offered is Year 2000
Compliant. Delivery is required within 30 calendar days after the date
of receipt of the order by the vendor. This will be a firm fixed price
contract. FOB is Destination. Payment will be made within 30 days after
delivery and acceptance by the VA. The following incorporated
provisions and clauses are those in effect through FAC 97-04 and are
applicable to this acquisition. 52.212-1, Instructions to Offerors --

shall provide an express warranty which at a minimum shall be the same
warranty terms, including offers of extended warranties, offered to
