This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. F2660098Q2026 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-05. Solicitation is unrestricted, open to all business. The SIC for this solicitation is 7699. Item description as follows: Nonpersonal service to perform preventive maintenance, calibration, and repair of Steris washers and sterilizer to include the following: 1) Six (6) 16x16" Stage III Washer/Sterilzer serial #'s 012949211, 013019205, 0130192304, 012949210, 012679205, 013279203. 2) Two (2)24x36" Stage II vac sterilizer serial #'s 012309202, 013459201 3) One (1) Reliance 3400 washer/sterilizer SN-013029203 4) One (1) 5 GPH U.P. still SN-011129214 5) One (1) 120 Cart Washer SN-3626392171 6) One (1)Sonic Energy Cleaner SN-0428992045 Maintenance agreement to include, but is not limited to the following; (6) six preventive maintenance inspections for the year, intervening repair calls during normal working hours, all parts under $200.00, and labor needed throughout the term of the contract. All other work is outlined in the Statement of Work (SOW). All equipment is located at Mike O' Callaghan Federal Hospital, Las Vegas, NV. The period to be covered is from 1 Oct 98 to 30 Sept 99, with four (4) option years. There are (5) five line items included in this solicitation (see bid schedule). The due date for submission of bids is 1700 PST 25 Sep 98. The provision at FAR 52.212-1, Instructions to Offerers-Commercial, applies to this solicitation. Award will be made to the responsible, responsive offeror whose offer will result in the lowest technically acceptable price. The government will evaluate options for award purpose by adding the total price for all options to the total price for the basic requirement for the total five-year period. The government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the government to exercise the options.
