This will be a personal services contract This requirement will be accomplished utilizing Best Value Commercial Item simplified acquisition procedures. Proposal(s) will contain both Technical and Business sections. The contractor will work 2week days during hours established by the Clinic Supervisor, 6 hours a day, 624 hours per year. Period of performance will be 01 January 1999 through 30 September 1999 with two option years. The Request for Proposals will be issued on or about 15 October 1998 and close on or about 16 November 1998. This procurement is a Small Business Set-Aside. The SIC code is 8049 and the small business size standard is $5.0 million average annual receipts for the preceding three fiscal years.
