Q -- PRIMARY CARE PROVIDER SOL RFP 613(CSC)-12-99 DUE 103098 POC germaine talbot The Government desires to contract with a primary care provider in Petersburg, WV servicing the veterans in the targeted counties of Grant, Hardy and Hampshire, WV. Contractor's facility to provide services must be physically located within the city limits of Petersburg, WV. Approximate issue date is 9/30/98. Offerors may propose a variety of unrestricted primary care models. The contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients. The contractor shall provide medical facilities, healthcare providers, medical equipment and supplies, limited pharmaceuticals, and all the administrative functions in accordance with the terms and conditions of the resultant contract. Compensation to the contractor will be made through a monthly capitation payment. All solicitation requests must be submitted in writing to the Contracting Officer.
