This is a combined synopsis/solicitation for commercial supplies/services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicititation; Request For Quotations (RFQ) are being requested and a written solicitation will not be issued. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-06. This procurement is set-aside for small businesses and the SIC Code is 5047 and the small business size standard is 100 employees or less. The following supplies are required: Item 0001 -- Vocare Implant System -- Qty 3 EA; Item 0002 -- Intradural Surgical Probe -- Qty 4 EA; Item 0003 -- Extradural Surgical Probe -- Qty 4 EA; Surgical Test Cable -- Qty 4 EA.
