synopsis/solicitation for commercial items prepared in accordance with
the format in FAR, Subpart 12.6 as supplemented with additional
information included in this notice. This notice constitutes the only
solicitation; quotes are being requested and a written solicitation
will not be issued. This solicitation is being issued as a Request for
Quotation number RFQ 688-048-0001. The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular number 97-05. The SIC code for this solicitation
is 5047. The small business size standard is 100 employees. Contractor
to provide a modular cardiac cath/angiography system comprised of the
following sub-systems: a General Electrical (Brand Name or Equal)
LUA-C Single Plane with 35mm cini (30,60,90 FPS) camera, (b) Digital
Fluoro 5000 however a DF4000 or DF3000 is acceptable with digital
fluoro subtraction angio system with PUCK UD for cut film application,
(c) Digital Imaging Processing: 1025 x 1025 bits; or a 512 x 512 bits,
(d) multiformat camera for use with 14' x 14' film or 14' x 17'
cassette, (e) Omega II Digital Steeping Table, (f) Medrad IV Injector
with programmable flow rate and time delay, (g) two (2) control room
monitors [minimum size 9"], (h) two (2) procedure room monitors
[minimum 17 inches], (I) Quinton Q Cath -- patients monitoring system
(j) three (3) day training for imaging service staff (k) twleve (12)
inch image intensifier or 9". To ensure operations of the modular
system, the system should evaluated by a qualified General Electric (or
equal's) technician prior to acceptance. The system will be needed for
six (6) months [November 1998 thru April 1999]. Acceptance time will
be 60 days from bid opening date. The following provisions apply to
this solicitation: 52.212-1 Instructions to Offerors-Commercial,
52.212-3 Representations and Certifications-Commercial Items, 52.214-4
Contract Terms and Conditions-Commercial Items, and 52.212-5 Contract
Terms and Conditions including clauses incorporated by reference under
Section (b)(6) 52.222-.26, Equal Opportunity (E.O. 11246), Affirmative
Action for Special Disabled and Vietnam Era Veterans (38 U.S.C. 4212),
52.222-36 Affirmative Action for Handicapped Workers (29 U.S.C. 793),
Buy American Act-Supplies (41 U.S.C. 10). A written notice of award or
acceptance of offer, mailed or otherwise furnished to the successful
bidder within the time for acceptance specified in the offer shall
result in a binding contract without further actions by either party.
Bidders responding to this announcement shall submit a price for this
system along with the information required in 52.212-1 and a completed
copy of provision 52.212-3 to VA Medical Center (688/90C), Room
1B-112, 50 Irving Street, NW, Washington, DC 20422 by October 15, 1998
at 4:30PM Local Time. Faxed bids will not be accepted. All responsible
