No telephone requests will be accepted. The Acquisition Service Center has a requirement for providing eyeglasses to veterans that require them and are eligible for assistance. This will be a multiple award proposal to several sites close to the seven medical centers which are located in Nashville, Mountain Home, Murfreesboro, Memphis, TN; Huntington, WV; and Lexington, Louisville, KY. The proposal requests single, bifocal and trifocal lenses, frames, eyeglass cases and lens treatments. This will be a firm fixed price contract with two (2) renewal option years. Eyeeglasses will be in strict conformance to the eyeglass prescription that is provided by the VA clinic team. The VA will require the contractor to delivery items ordered under this contract no later than 7 calendar days after receipt of a delivery order. Only writtenrequests will be honored.
