This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is being issued as a request for quotation. The solicitation number is RFQ 565-4-99. Quotations are due by the close of business October 19, 1998. The VA Medical Center, Fayetteville, NC intends to award a firm fixed price contract for quarterly preventive maintenance inspections on its Ohio Medical Anesthesia Machines and all components including ventilators (excluding vaporizers) and its Respiratory Gas Monitors. Contract period will be from the date of award through September 30, 1999. Award will be made for the base period with four option years. The total duration will not exceed five (5) years pending availability of funds. Quotations shall be submitted on an all or none basis and be valid for 90 days. Partial quotations shall be rejected. Contractor shall provide four (4) inspections during the contract period (October, January, April, July). Inspections shall be scheduled in advance with the Contracting Officer's Technical Representative. All work will be performed during normal hours of coverage, which is Monday through Friday from 8:00 a.m. to 4:30 p.m., excluding federal holidays. Contractor shall provide all labor, tools, materials and equipment to perform inspections. Preventive maintenance shall include, but need not be limited to, inspection, cleaning and any adjustments necessary to maintain the equipment in good operating condition. Equipment shall be inspected and function in conformance with manufacturer's specifications. All work shall be performed by competent, experienced technicians who are qualified to work on specified equipment. All work shall be performed in accordance with good commercial practices. Responses should be based on quarterly cost for all equipment to be inspected. Submit total quarterly costs for all items in Requirement A.
