66 -- PROCUREMENT OF TDX FLX THERAPEUTIC DRUG ANALYZER SOL M6-Q1-99 DUE 102698 POC Jane Brusveen (708) 786-7659 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This procurement is considered for noncompetitive contracting action. Solicitation M6-Q1-99 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-08. For informational purposed only: The Department of Veterans Affairs, Office of Acquisition and Materiel Management, National Acquisition Center, Hines, IL, intends to contract on behalf of the U.S. Air Force AFMLO at Ft. Worth, Texas, 1 (one) TDx FLx Therapeutic Drug Analyzer from Abbott Laboratories (Abbott) with all appropriate Abbott software updates. This procurement is considered for noncompetitive contracting action due to the fact that the supplies for this analyzer have already been purchased by the USAF and Abbott supplies can only be used with the Abbott TDx FLx Analyzer because of the packaging specifications of the supplies. System description specifications are: sample carousel capacity is 1 to 20 samples; reagent carousel is 1 to 8 TDx FLx wedge reagent packs; sample volume is 50 to 500 ul; carry over is <1.5% at concentrations greater than or equal to the highest calibrator, unless otherwise specified; intensity/stability is better than 0.1% over the duration of an assay, and polarization range is 0 to 500 mP. Analyzer must have all appropriate Abbott software updates. Delivery will be FOB Destination and acceptance approximately 60 days after receipt of order to AFMLO/OL-2, 501 Felix Street, Building 6, Ft. Worth, TX 76115. Provisions 52.212-1, Instruction to Offerors -- Commercial, 52.212.4 Contract Terms and Conditions -- Commercial Items, and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items apply. Provision 52.212-2 Evaluation -- Commercial Items applies using the following factors listed in their order of importance to evaluate the offer: (i) technical capability of the item offered to meet the Government requirement; (ii) price; (iii) past performance -- 2 (two) recent customers who purchased the TDx FLx Analyzer listing a point of contact, address, purchase order number, and telephone number. Also, a completed copy of provision 52.212-3, Offeror Representations and Certifications -- Commercial Items, must be submitted with quote. Only devices that have received Premarket Notification Approval from the U.S. Food and Drug Administration (FDA) shall be considered for award under this solicitation. Offeror must furnish proof of FDA 510(k) Premarket Notification approval with this Request for Quotation. The offeror shall enter, in the block with its name and address on the RFP, the annotation "DUNS" followed by the DUNS number that identifies the offeror's name and address exactly as stated in the offer.
