This procurement shall be processed in accordance with the Simplified Acquisition Procedures of Part 13 of the Federal Acquisition Regulations. The purpose of this Purchase Order is to provide lactation support services to all CDC/ATSDR and Commissioned Corps female employees participating in the New Mother Lactation Support Program. Services shall be provided at various CDC/ATSDR locations on a nationwide basis. The contractor shall furnish required breast pumps manufactured by Medela, or equal, training, consultation services, and promotional/training literature. Offers submitted must include data to demonstrate experience in providing lactation support services as described in the Statement of Work. The period of performance is 12 months with one 12-month option period. This is a simplified acquisition, 100% small business set-aside, issued as a request for quotation (RFQ), 1999Q00013. Quotes submitted from other than small business will be rejected. The Standard Industrial Code (SIC) is 8748 and the size standard is 500 employees. Interested parties should request RFQ 1999Q00013.
