This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in FAR, Subpart 12.6 as supplemented with
additional information included in this notice. This notice constitutes
the only solicitation; quotes are being requested and written
solicitations will not be issued. (i) Solicitation DAHA-98-T0120 is
issued as a Request for Quote. Quotes are requested and a written RFQ
will not be issued. (ii) The solicitation document incorporates
provisions and clauses that are in effect through Federal Acquisition
circular (FAC) 97-08 and Defense Acquisition Circular 91-18. It is the
contractor's responsibility to be familiar with all applicable clauses
and provisions. (iii) This acquisition will be a 100% Small business
Set Aside, the Standard Industrialization Code (SIC) is 3842 with a
Size Standard of 500 employees. (iv) The 128 ARW Contracting Office has
a requirement for one (1) each Confined Space Entry Monitoring System.
(v) The following is a brief summary of the required system (1) System
shall consist of a portable monitoring center with the following
functions: data acquisition and reporting of wireless telemetric data,
alarm generation of abnormal conditions, remote alarm indication, and
voice communications. (2) System shall include portable monitors with
wireless telemetric data on the condition and well-being of confined
space personnel, and integration with various types of environmental
monitoring devices (such as gas detectors). (3) System shall be
designed in lieu of using gas testing personnel as well as tank
monitors allowing worker to go safely into a confined space alone. (4)

your company name, address, point of contact, and voice and fax phone
numbers. Monitoring system to be delivered FOB Destination to General
Mitchell IAP, Milwaukee, WI 53207. Warranty: The contractor or
manufacturer standard commercial warranty shall apply. Unless otherwise
specified in the purchaseorder, the supplier is responsible for the
performance of all inspection requirements and Quality Control. (vi)
The following FAR provisions and clauses are applicable to this
procurement: 52.212-1, Instructions to Offerors-Commercial Items is
hereby incorporated by reference. (vii) Solicitation provision at FAR
52.212-2, Evaluation-Commercial Items, is hereby incorporated by
reference. The Government will award a Purchase Order resulting from
this solicitation based on the following: price, technical capability,
technically acceptable, responsible, responsive, past performance, and
delivery schedule in compliance with the requirements of this
solicitation and all referenced clauses with technical capability being
the most significant factor. (vii) Offerors are reminded to include a
completed copy of the provision at 52.212-3, Offeror Representations
and Certifications Commercial Items and 252.212-7000, Offeror
Representations and Certifications Commercial Items with its offer.

incorporated by reference. (ix) Clause 52.212-5, Contract Terms and
Conditions Required to Implement Statutes or Executive Orders
Commercial Items, is hereby incorporated by reference. (x) Signed and
dated quotes must be submitted to 128 ARW Contracting Office, 1919 E.
Grange Avenue, General Mitchell IAP (ANG), WI 53207-6141, ATTN: James
Ensign not later than 3:00 PM (CST) 30 Oct 1998. (xi) Contact James
Ensign at 414-747-4466, Fax 414-747-4181, no collect calls. A completed
copy of the provision at 52.212-3, Offerors Representations and
Certifications Commercial Items, must be submitted with the quote.
Offerors submissions shall comply with the instructions set forth in
this solicitation and FAR provision 52.212-1, Instructions to Offerors
Commercial Items and shall include, in accordance with FAR 52.212-1,
the following: Technical description of the items being quoted in
sufficient detail to evaluate compliance with the requirements of the
solicitation; the terms of any express warranty, price and any
discounts; delivery terms; completed copy of the representations and
certifications at FAR 52.212-3, past performance information IAW this
clause, and shall include all other applicable information required by
