JOINT SERVICE AIRCREW MASK (JSAM)-REQUEST FOR TECHNICAL DATA As a result of a request from industry for technical data from this office, the following information is provided for your use in becoming certified to request technical data from the Government with restrictive markings. First, a contractor must fill out a DD Form 2345-Militarily Critical Technical Data Agreement, which can be obtained from the Washington Headquarters Services forms homepage at http://web1.whs.osd.mil/icdhome/DDEFORMS.HTM. Second, this form must be completed and sent to: United States/Canada Joint Certification Office Defense Logistics Information Service (DLIS) Federal Center 74 N. Washington Ave. Battle Creek, MI 49017-3084 Within 5 working days of receipt of the form, DLIS will process it and, if approved, place the contractor on the Certified Contractor Access List (CCaL). Once a contractor has been approved and their DD Form 2345 on file, a company can get the technical data directly from the source. The Government will verify through CCaL that a contract has in fact been approved to receive technical data. The certification is good for 5 years and can be renewed.
