Dubose, 770-488-9486 This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation number is TIRSE-99-Q-00006 and is being issued as a
request for quotations. The solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-06. This requirement is set-aside for small businesses for
which the Standard Industrial Classification Code 8011. The Internal
Revenue Service requires part-time professional medical services at the
Atlanta Service Center, 4800 Buford Highway, Chamblee, GA, to include
a diagnostic care of a preventative nature only, with consultation
services as needed. Period of performance will be a base and option
periods not to exceed 36 months. CLIN 0001 Physician Services -- BASIC
SERVICES The contractor shall provide on-site medical services to the
IRS nursing staff and, therefore, to IRS employees l ocated at the
Center. Services shall be performed 4 hours one day per week, with the
primary tour of duty from 7:30 am -- 11:30 am. The contractor shall be
available via pager or cellular phone during clinic hours The
contractor may be required to schedule hours in the afternoon about
once every two to three months to perform services for employees
working on the night shift. Contract services will consist of the
following: (1) Conduct an estimated 200 comprehensive annual physical
examinations as schedule by IRS. For female employees examinations
shall include a pelvic examination and pap smear, if requested. For all
male employees, rectal-prostate examinations shall be included. (2) In
conjunction with conducting physical examinations, the contractor
shall review and make recommendations to employees on results from the
following tests (which have been performed by the IRS nursing staff or
an outside source): Laboratory reports -- blood and urine,
Electorcardigram, Visual acuity, Audiogram, Pulmonary function,
Hemoccult slides, and P.P.D. (tuberculin skin test).(3) Provide medical
direction for the IRS registered nursing staff. (4) Provide periodic
in-service education for the professional nursing staff and employees
on current medical issues and other requested topics. This will be
required approximately 4 to 6 times per year. (5) Provide oversight in
the Health Unit while immunizations (such as flu shots) are
administered by nurses.(6) Review and make appropriate changes to the
IRS Health Unit medical procedure book and establish standing orders.
(7) As requested, review questionable Office of Workers Compensation
Program (OWCP) claims. Discuss the claim and the medical documentation
with the employee's physician, as needed, and report findings to the
IRS Labor Relations (LR) office. This is required very infrequently.
(8) Review medical documentation on absences due to illness which are
received from employees located in IRS Atlanta Host Site offices, other
than the Atlanta Service Center. This review is necessary to determine
the adequacy of the documentation. Upon completion of each review, the
contractor must provide necessary feedback to the employee's manager.
This requirement is anticipated to be approximately 80 such requests
per year.(9) As requested by the nursing staff, assist with reviews of
medical documentation from Atlanta Service Center employee's physician
regarding documentation submitted.(10) At the nurse's discretion, may
be requested to assess an employee to determine the urgency of a
specific medical complaint, or consult on a job-related accident or
emergency occurring during physician's tour of duty hours. (11) Provide
to the nursing staff on-going guidance and in-service education on
occupational exposure to blood-borne pathogens.(12) Advise management
on questions of physical qualifications, health and safety, and other
aspects of occupational medicine.(13) As requested by the nurse(s),
provide advice outside tour of duty hours via telephone. A maximum of
10 such "advice calls" per month shall be provided at no additional
charge to the Government. This requirement will cover unusual
circumstances where a question arises concerning employee treatment for
other than an emergency situation. Calls exceeding 10 per month will be
handled under additional services (14) Provide other related services
which may arise during contractor's tour of duty hours. These are
sporadic or infrequent requests for advice of a routine nature.
Additional services are defined as being optional services specifically
requested by the Government on an "as needed" basis.These services will
cover requests for telephone advise in excess of the 10 "advice calls"
per month provided under Basic Services. CLIN 0002 -- ADDITIONAL
SERVICES -- to be performed, as requested, in addition to basic
services. The contractor is responsible for providing the professional
services listed above, and forpossessing all licenses, certificates
and insurance necessary for performance under this contract. The
Government assumes no liability for the acts of the contractor with
respect to the diagnosis or treatment of an employee's health
problem(s). Services performed by the contractor will not be subject to
the supervision or control of any employee of the Government with
respect to the method used to perform the services. However, Government
reserves the right to request medical opinions of other specialists.
The IRS shall provide a fully equipped health unit at the Atlanta
Service Center where the Contractor will render professional services.
The IRS Health Unit will have a staff of occupational health nurses.
Typing and other clerical services will be provided by the IRS. The IRS
will provide test kits for all pap smears (standard test kits with
plastic speculum), and an IRS-contracted laboratory will be responsible
for the blood and urine tests; EKGs will be provided and paid for the
IRS.The contractor will be provided a reserved on-site parking space at
no charge. Nothing in this contract, either by intent or implication,
is to be construed as establishing an "employer/employee' relationship
between the Government and the contractor. Within 5 calendar days
following contract award, contractor shall submit a work schedule to
the for review and approval. In this work schedule, the contractor
shall set forth the day during the week which will be spent performing
contract services. Any proposed change to the established work
schedule must be submitted for approval a minimum of 10 calendar days
in advance of such proposed change. The change must be mutually agreed
upon between the contractor and the IRS. The total weekly requirement
is fixed at 4 hours, to be performed during one scheduled day per
week.The contractor will be entitled to take three 4 hour weeks of
vacation at his/her own expense. Vacation schedule must be submitted
for approval a minimum of 30 calendar days in advance. Any unscheduled
absence from work will result in a prorated deduction from the price
paid to the contractor. The contractor will not be required to work and
will not be paid for the 10 Federal holidays. The contractor shall
maintain the appropriate Licensing and Insurance to practice within the
state of Georgia. Contractor must provide references of good clinical
work and ethical conduct. Contractor shall be required to provide proof
of medical liability insurance in accordance with the requirements of
Federal Acquisition Regulation (FAR) 52.237-7, Indeminification and
Medical Liability Insurance, prior to award of contract. The provisions
at FAR 52.212-1, Instructions to Offerors- Commercial items,
specifically paragraphs (a), (b)(1), (b)(6), (b)(7) and (b)(11);
52.212-2, Evaluation -- Commercial Items, apply to this solicitation.
FAR 52.213-3, Offer Representations and Certifications-Commerical Items
