synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. (II)This solicitation is a Request for
Quotation, number 117-0557. (III)The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-07. (IV)This solicitation is 100% set aside for
small business. The sic code is 8011. Small business size standard is
$5.0 million. (V)Item 1, Radiological Diagnostic service, 1 session per
month. A session is a two (2) hour period. Item 2, Radiological
Interpretation, 84 studies per month. The quantities are estimated for
both items. The contractor shall include the cost of shipping in the
per study cost. This is an option year contract, consisting of a base
year and three option years. The services and quantities are the same
for each year. (VI)a radiologist is required to perform and interpret
flouroscopic examinations on the inmates housed at the Federal
Correctional Institution(FCI) and the Federal Detention Center(FDC)
Milan, Michigan. Flouroscopic examinations include but are not limited
to barium enemas, upper & lower GI's, and arthroscopy. The institution
will send all films for interpretation to the contractor on a weekly
basis. The contractor will provide typewritten reports and legible
signature on the the form provided and return the studies within one
week. The contractor will be available for same day readings in the
event of an emergency study or phone consultation. (VII)The service
will be conducted during the normal hours of operation, Monday-Friday,
7:30am-4:00pm. (VIII)Provision 52.212-1, Instructions to
Offerors-Commercial Items, applies to this acquisition. (IX)Award shall
be based on pricing and related factors only. (X)Offerors must include
a completed copy or the provision 52.212-3, Offeror Representations &
Certifications-Commercial Items. (XI)Clause 52.212-4, Contract Terms
& Conditions-Commercial Items, applies to this acquisition. (XII)Clause
52.212-5, Contract Terms & Conditions Required to Implement Statutes of
Executive Orders-Commercial Items, applies to this
