The VA Roseburg Healthcare System (VARHS), located at 913 Garden Valley Boulevard, Roseburg, Oregon requires primary care on an outpatient basis for approximately 570 Curry, Oregon and Del Norte, California, veterans currently receiving primary care through the Roseburg system. The community based provider will be responsible for providing and managing eligible veterans' primary medical care. Patients requiring specialty care and/or hospitalization will be referred to Roseburg unless otherwise designated. To be considered for contract award, the offeror must: (1) demonstrate existence as a medical busienss entity providing primary care services for a minimum of two years as of the date of proposal submission; (2) provide certification that all providers for the required services (including services to be provided by subcontract arrangements) are licensed by the appropriate governing body; (3) have a facility located in Brookings or Crescent City or within 5 miles of either city along the Highway 101 Coast corridor. Evaluation criteria are described in the Solicitation. It is anticipated that contract award will be towards the latter part of December, 1998 with services beginning within 60 to 90 calendar days of contract award. The term of the contract will be for one year with a one-year option to extend. A commercial item request for proposal will be issued on or about November 5, 1998.
