R -- PROVIDE HEALTHCARE CONSULTANT SERVICES SOL RFQV15-99-3 DUE 112098 POC Glenn Carpenter, Contract Specialist, 913-758-4100 The VA Heartland VISN 15 is seeking consultant services required to assist the network in developing a Request for Proposal "RFP" Document that can be used as a model for obtaining healthcare provider organizations that can provide services to VA beneficiaries. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-05. Work will consist of providing consultant services necessary to assist VISN 15 in developing a Request for Proposal that can be used as a model for obtaining healthcare providers that can provide service to VA beneficiaries in VISN 15 when required. The following will be provided by the contractor: 1. One sample Request for Proposal "RFP" including recommended standard contractual language and criteria for the evaluation of offers, including technical and pricing. The Request for Proposals (RFP) will be developed by the contractor for provider services to be offered to eligible VA beneficiaries in the following geographical regions: St. Louis, MO; Kansas City MO; and Wichita, KS $______________(all inclusive fee) 2. Designing the format and assisting with one pre-bid conference with prospective providers including answering questions raised by potential offerors. $______________(all inclusive fee) 3. Developing a document to be used for the pre-award survey of prospective provider offerors and providing assistance when required. $______________(all inclusive fee) 4. Develop strategies and guidelines for negotiating contracts to be awarded including the scope of services provided, the cost, access to care and other contract requirements. $______________(all inclusive fee) 5. Help evaluate proposals with VISN 15 staff or provide additional onsite support when required. $______________/hr. $_____________/round trip air fare to Kansas City $_____________/per day per diem Proposals will be evaluated for both technical merit and price reasonableness. The Technical Proposal format, which follows, has been included to assist you in preparing a complete technical proposal. In order to provide full consideration of your qualification for contract award, you are encouraged to ensure that the information furnished in support of your technical proposal is factual, accurate and complete. You may provide additional technical information you believe will enhance your proposal; however, overly elaborate presentations are not desired. Failure to provide the information requested may render the offer's proposal technically unacceptable. The Government reserves the right to verify any information provided for evaluation purposes. TECHNICAL PROPOSAL FORMAT I. Personnel Qualifications (1) Provide your Management Qualifications (2) Provide your Staff Qualifications II. Understanding the Requirement. (1) Indicate the proposed method to provide the services. Indicate the estimated number of staff hours proposed for each work element. (2) List the personnel to be used and their experience for providing the services required. III. Past Performance. List at least six (6) references or experiences in providing an equal or similar services and that implementation has been completed. List three references that you prefer we call. Please include name, address, telephone number and point of contact. References should be within the last three (3) years. The following incorporated provisions and clauses are applicable to this acquisition: 52.212-1 Instructions to Offerors -- Commercial Items; 52.212-2, Evaluation-Commercial Items, para (a) is filled in as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
