Q -- ADMIXING, LABELING & DELIVERY OF PIGGYBACK AND HYPERALIMENTATION PRODUCTS. SOL RFP 600-016-99 FF DUE 010899 POC Frank Fullbright, Contracting Officer, (NBC/MP), (562) 961-8065 Seeking sources to provide Customized Admixing, labeling and delivery of piggyback and hyperalimentation Products (Ampicillin, Cefotaxine, Nafcillin, Cefazolin, Pipercillin/Tazobactam, Ceftizoxine, Ceftazidime, Vancomycin, Amikacin, Clindamycin), Ranitidine, Amino Acid/Dextrose, to the VA Medical Center, Long Beach. Medications required to admix the piggyback orders will be supplied by the VAMC, Long Beach. Contractor must have the capability with the VAMC Long Beach's DHCP Computer System for transmitting orders directly to the Admixture Service. Contractor to provide all materials used in preparing the admixture solutions such as transfer sets, TPN clips, Needles, Syringes, Seals, swabs, gloves & sharp containers. Piggyback and hyperalimentation solutions will be in compliance with the American Society of Hospital Pharmacists (ASHP) Guidelines & conducted in a Class 100 or Comparable Clean Room. Contractor shall label & number the I.V. Products with a label matching those currently @ VAMC, Long Beach. Deliveries to be made three time daily, seven days a week, within four hours after receipt of order. Usage records for each product admixed must be provided monthly. In addition, VAMC Seattle, WA, VAMC Denver, CO, and VAMC Hines, IL. will participate in this consolidated contract. Award to be made for a period of one year from date of award with the option to award for one additional year. RFP 600-016-99FF will issue o/a 12/9/98 with proposal due on 1/8/99 by the close of business (4:00PM local time). All requests for solicitation RFP 600-016-99FF must be made in writing.
